Dear Mr. Conner:
On September 26, 2024, a site inspection was conducted for the B.V. Hedrick-Lilesville Quarry facility
located at SR 1704, Lilesville, Anson County, North Carolina. A copy of the Compliance Inspection Report
is enclosed for your review. Kevin Tobin, Assistant Land Manager, was present during the inspection and
his time and assistance is greatly appreciated. The site visit and file review revealed that the subject facility
is covered by NPDES Stormwater General Permit NCG020000 under Certificate of Coverage NCG020220.
Permit coverage authorizes the discharge of stormwater from the facility to receiving waters designated as
Savannah Creek, class WS-IV waters in the Yadkin River Basin.
As a result of the inspection, the facility was found be compliant with the conditions of the NCG020000
permit. Please refer to the enclosed Compliance Inspection Report for additional comments and
observations made during the inspection.
Please be advised that violations of the NPDES Stormwater General Permit are subject to a civil
penalty assessment of up to$25,000 per day for each violation. If you or your staff have any questions,

Inspection Summary:
Melissa Joyner and Denise Bruce met with Kevin Tobin, Assistant Land Manager. There was no Stormwater Pollution
Prevention Plan (SWPPP)developed which would be subject to General Permit NCG020000 conditions Part D:Inactive
Mines/Dormant Status, D-1 through D-6. There were no required, documented annual inspection and stormwater control
measures (SCM) semi-annual inspections. Semi-annual inspections would involve looking at the permanent groundcover to
determine if there were any issues with erosion. The annual on-line self-certification about the current Dormant Status
conditions had not been conducted. Please go to the following link under"Dormant Status" to initiate this process.:

inspected on 6/6/2024 and was inactive at the time of the inspection. It was not inspected during this inspection. Please
also refer to the conditions of the letter issued on 11/15/2013 "Exemption from Monitoring During Shut-Down (Site
Dormancy)". This letter should be retained in the SWPPP.
The facility is not in compliance with the conditions of Stormwater General Permit NCG020000 because there was no
SWPPP developed which would include the aforementioned required information. There has also not been an annual on-line
self-certification about the current Dormant Status conditions.
