your review and approval of the modification for existing stormwater management permit(SWA000114)for the
subject project.
We are enclosing the following:
1. Two copies Sheets Cl (folded to 8.5"x 14"), as issued, "Issued for Permit Modification"dated,
September 17,2024;
2. One original of the signed and notarized Permit Application(Major Modification);
2. One original of the Supplement-EZ Cover Page;
3. One original of the signed and notarized O&M Agreement(low density)and required supplement form;
4. A copy of the recorded plats for each phase of the development;
5. A draft copy of the Deed Restrictions and Protective Covenants;
6. A check in the amount of$1,000.00 made payable to NCDEQ as payment of the plan review fee.
