Thank you for paying the review fee. The Division has determined that your application is incomplete and cannot be
processed. I attempted to send a request for additional information to brysontrucking@vahoo.com as that email
address is listed as the primary contact on the application. However, that email was returned as undeliverable as the
mailbox was not found.
The application will remain on -hold until all of the following information is received:
Hubert Bryson signed the application. Title 15A NCAC 02H .0502(a)(11) requires that if an agent signs the
application, an agent authorization letter must be provided. Please provide an agent authorization letter form
signed by Jared Zimmer granting Hubert Bryson signature authority. A sample form is attached. [15A NCAC 02H
.0502(a)(11)]
The Division is unable to identify the location of the proposed project based on the information provided. Please
provide a map that clearly shows the location of the property, the numerical portion of the property address on
New Trillium Way, or more accurate GPS coordinates. [15A NCAC 02H .0502(a)(6)]
Please provide a detailed site plan showing property boundaries and proposed locations of vegetation clearing,
structures (buildings, retaining walls, docks, impervious surfaces, etc.), rip rap, excavation or dredging below full
pond/normal water level elevations, and construction access corridors. You may use the attached diagram or a
property plat. [15A NCAC 02H .0502(a)(9)]
Pursuant to Title 15A NCAC 02H .0502(e), the applicant shall furnish all of the above requested information for the
proper consideration of the application. Please provide your response by October 10, 2024. If all of the requested
information is not received, the Division will be unable to approve the application and it will be denied as incomplete.
The denial of this project will necessitate reapplication to the Division for approval, including a complete application
package and appropriate fee.

DWR has not received payment of the application fee for this project. Unless timely payment is received
the application will be returned as incomplete. Attached is the original request for payment with
payment instructions. Please coordinate so the fee is only paid once as double payments can cause
further delays. Let me know if you have any questions or concerns.
