Please upload the "Closeout of Land Disturbing Permit," from Buncombe County. It will have the project
name, the erosion control permit number, and the close out termination language.
On the NOT Certification Form, the legally responsible organizational entity should be Duke Energy
Carolinas, LLC and the legally responsible person should be Jaclyn Whiteman. Either have Jaclyn
Whiteman sign the NOT Certification Form and submit the eNOT, or a delegated individual from the Duke
Energy Carolinas, LLC to print their name and title on the "Print Name and Title of Signed if Authorized"
line of the NOT Certification Form, sign the form using an original wet signature, and upload the updated
NOT Certification form.
