Report". The report summarizes the findings of a recent inspection of the project's stormwater
controls conducted on 7/5/07 to determine compliance with Stormwater Permit Number
SW8960710 issued on 2/25/1997.
The required actions that are necessary to bring this site into compliance are noted under the
Comments section of the report. Please inform this Office in writing within fifteen (15) days of

This facility was purchased by Glen &Ann Kornegay The Name was changed to Glencare of Holly Ridge
in Sept. 06 210 Limestone Rd.

I met with the property manager Belinda DeLuca and gave her a copy of the permit and the 0 & M agreement with Mr.
Brewer and explained that the permit could not be transferred, a formal request for a name change must be received and
signed by both parties.
The pond was in need of maintenance. The vegetative growth around the pond and inside was such that I could not see
the parts of the pond. I could barely see the ouffall structure which was covered with grass and cattails. The large woody
growth should be cut and the amount of cattails inside the pond reduced to no more than 50% coverage. When these are
cut, they should be removed, and not allowed to decompose in the pond.
There was some slight erosion on the top of the embankment. Some minor reseeding is necessary.
The outlet bypass was in place, however, it appeared to have been bermed up with rock(rip-rap)on the outside of the
fenced area. This may not allow a free flow if the situation was needed.
The permit expired in February.
