This Office would like to thank you for all of your assistance and cooperation provided as a result of this inspection. As discussed in more detail in this report, you are required to adhere to the requirements of the NPDES Permit including proper operation and maintenance, record retention, annual effluent sampling, payment of annual fee, etc. This Office had determined through the research of property records and email correspondence that you had become the new owners of the subject property in March/April 2022. Your timely response to convert the permit to you (following previous notification) is appreciated; however, there are actions that need to be initiated to comply with this permit. Please note that the failure to comply with this permit can subject you to potential civil penalties up to twenty-five thousand dollars ($25,000.00) per day, per violation. In addition, the failure to pay the annual fee can also potentially subject you to permit revocation. It is requested that a written response be submitted to this Office by September 27, 2024, detailing the actions taken to address all issues discussed in the attached inspection report. In addition, Part I, Section D (Operation, Maintenance & Records)of the subject permit has specific operation and maintenance requirements that need to be reviewed and complied with. This Office recommends the permittee to contract a company experienced in the operation and maintenance of these types of treatment systems. Effluent Pipe Yes No NA NE Is right of way to the outfall properly maintained? ❑ ❑ ❑ ■ Are the receiving water free of foam other than trace amounts and other debris? ■ ❑ ❑ ❑ If effluent (diffuser pipes are required) are they operating properly? ❑ ❑ ■ ❑ Comment: The discharge outfall for the wastewater treatment must be located and properly maintained. The outfall could not be located during the inspection; however, no indication of wastewater solids or foam were observed in the receiving stream in the direct vicinity of the treatment system.
