NORTH CAROLINA Director Environmental Quality September 9, 2024 Carlos Hernandez 6212 Old Kemp Road Durham,NC 27703 Subject: Compliance Evaluation Inspection Single Family Wastewater Treatment System 6212 Old Kemp Road Certificate of Coverage NCG551657 Durham County Dear Mr. Hernandez: On August 13, 2024, Danny Smith from the Raleigh Regional Office of the Division of Water Resources (DWR) visited your single-family residence (SFR)wastewater treatment system to evaluate compliance with the subject General NPDES Permit. This wastewater treatment system is located at 6212 Old Kemp Road, in Durham County North Carolina and is currently permitted to Carlos Hernandez. Our records indicate the wastewater treatment system consists of a septic tank, EZ treat wastewater system(EZ treat pod, recirculating tank, control panel), UV disinfection, and effluent discharge pipe. General NPDES Permit NCG550000 and Certificate of Coverage (COC)NCG551657 authorize this discharge of domestic wastewater from your treatment system to receiving waters designated as unnamed tributary to Rocky Branch in the Neuse River Basin. This authorized discharge is in accordance with the effluent limits and monitoring requirements established within the General Permit. The subject visit and file review revealed the following: 1. Pumping the septic tank: Septic tanks should be pumped out approximately every five years or when the solids level is found to be more than 1/3 of the liquid depth in the septic tank compartment, whichever is greater. A pumping company can check the status periodically and determine when pumping is required. 3. Discharge outlet location. The permittee is required to conduct a visual review of the outfall location at least twice each year to ensure that no visible solids or other obvious evidence of system malfunctioning is observed. Any visible signs of a malfunctioning system shall be documented, and steps taken to correct the problem. Please take the necessary steps to ensure the discharge outlet remains identifiable/visible and accessible. 4. Analyzing the effluent: Part 1. C.,Effluent Limitations and Monitoring Requirements, within General Permit NCG550000 requires a permittee to sample and analyze the effluent leaving the treatment system. The required frequency of wastewater sample collection and analysis is once per year(annually). The General NPDES Permit requires the permittee to retain records associated with sewage disposal activities for a period of at least 5 years and to have the data available for review. Is recommended that the UV bulb be replaced annually or at the frequency as recommended by the manufacture. If you have not already collected the effluent for this year,please collect a representative sample of the effluent and have it analyzed by a certified commercial laboratory. If no wastewater flow is observed from your system,please document that information for your records and use during the next system inspection. Part II Section B.14 of General Permit NCG550000 requires the permittee to "pay the annual administering and compliance monitoring fee within thirty days after being billed by the Division." A review of the billing records for this facility indicates that the permit annual fee has not been paid for 2023 and 2024. Attached to this letter are the invoices for2023 and 2024. Please pay these invoices within 45-days of receipt for this correspondence. 6. Attached are copies of the General Permit NCG550000 in both English and Spanish for your reference. Thank you for your attention to these matters. Please continue to periodically inspect the wastewater treatment system ensuring the treatment components are always maintained and in good operating order. You are also reminded to maintain all monitoring data and associated maintenance records onsite for a minimum of five years and have this material available for inspection.
