Michelle,
There are a couple of options in our current system:
(1) The permittee can apply for a new COC and let us know it is specifically to correct an address typo. Since it
was identified within 60 days,we will waive the fee for a new eNOl and just apply the original one. Once
that's obtained, you submit a Notice of Termination (NOT)for the original COC. This option gives the
permittee a new COC with a revised address.
(2) Don't worry about revising the COC and just submit information through our Modification form. The
permittee would have to submit a Mod request and explain what information they are adding to the file
(basically a record that the address on the COC and in the original application was incorrect). Those
records will be available in the repository if there's ever a question about it. No new COC is issued with that
route.
I hope this information helps. Next year we should have a new system with more options for this type of correction
/revised COC issuance.
Best regards,
Bethany Georgoulias(she/her)

Good afternoon:
I made a clerical error on this Certificate of Coverage. The correct address is 2520 W Cumberland St. Is there
a way to fix that error on your end?
Thank you,
