To Whom It May Concern:
Pursuant to the Maryland State Department of Assessment and Taxation Records,
Articles of Incorporation were filed for an entity known as Boynton Beach
Campus, LLC on August 6, 2004; then on December 21, 2005, Articles of
Amendment were filed to change the name of the entity to Charlotte Campus,
LLC.
The above -referenced permit (the "2008 Permit") was issued on September 29,
2008 under Charlotte Campus, LLC, On March 29, 2011, Articles of Cancellation
were filed to dissolve Charlotte Campus, LLC.
Windsor Run, LLC is the operator and landowner of the Windsor Run Retirement
Community. Since the 2008 Permit was issued, Windsor Run has filed additional
permits by phase and, essentially, duplicate permits that cover this same 2008
Permit.
Since Charlotte Campus, LLC was dissolved in 2011, we are hereby requesting
that the 2008 Permit be closed out.
