Dear Mr. Crawford:
On August 28, 2024, Division of Water Resources (DWR) Raleigh Regional Office (RRO) staff Jesse Barnes conducted
a routine compliance evaluation inspection of the single family wastewater treatment system located at 5606
Wendell Rd, Chapel Hill, NC 27517 in reference to NPDES General Permit NCG550000 and Certificate of Coverage
(COC) NCG551739. No one was present at the residence during the inspection and attempts to contact permit
affiliates using information on file were unsuccessful, so DWR staff were unable to determine the system's
permit compliance status (see Item 2, below). A review of the subject permit and internal records was
completed. Any determined permit violation(s) and item(s) of concern are listed below, along with other
observations. Inspection findings are detailed in the attached Water Compliance Inspection Report.

North Carolina General Statute (NCGS) 143-215.1 (a.) (2.) states that no person may operate a treatment
works or disposal system unless that person has received a permit.
Durham County property records indicate the subject facility is owned by Robert H. Crawford IV. As the
property owner, you are also the owner of the existing single-family wastewater treatment system, which
treats the domestic wastewater from the residence and releases the effluent to waters of the State.
According to DWR records, a Permit Name/Ownership Change Form has not been submitted for the subject
facility. The permit issued for this facility still belongs to the facility's former owner, Mona P. Crawford.
To comply with NCGS 143-215.1 (a.), which requires a person to obtain a permit to make an outlet into
waters of the State, you must complete and submit the attached NCG550000 Permit Name/Ownership
Change Form to DWR.
2. DWR staff were unable to contact the permit affiliate or system owner using contact information on file.
Please contact Jesse Barnes at [phone] or [e-mail] within fifteen (15) days of
receipt of this letter, to provide additional information necessary to determine the facility's permit compliance
status.

Other Observations
The subject facility is a single family wastewater treatment system located at 5606 Wendell Rd, Chapel Hill, NC
27517. Version 2.0 of Certificate of Coverage NCG551739 for NPDES General Permit NCG550000 became effective
October 06, 2021, and will expire at midnight on October 31, 2025. The subject facility is authorized to discharge
wastewater treated in accordance with the subject permit via outfall 001 to the nearby unnamed tributary (UT) to
Little Creek (Stream Index: 16-41-1-15-(0.5)), classified as Water Supply IV (WS-IV) and Nutrient Sensitive Waters
(NSW) of the State within the Cape Fear River Basin (Sub -basin 03-06-06).
During the inspection, no obvious signs of system failure were observed.
Within fifteen (15) business days of receipt of this letter, please complete the following items:
1. Complete and submit the attached NCG550000 Permit Name/Ownership Change Form.
2. Contact DWR staff Jesse Barnes at [phone] or [e-mail] to provide
additional information necessary to determine the facility's permit compliance status.
Failure to complete the above requested items may result in further enforcement actions.
Please be aware that the Division may take enforcement action against any individual who fails to comply with the
requirements of all applicable permits, regulations, and/or statutes. Pursuant to NC General Statute (NCGS) 143-
215.6A, a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed against any
person who violates or fails to act in accordance with the terms, conditions, or requirements of any permit, or
applicable regulations, issued pursuant to G.S. 143-215.1.
