Dear Ms. Adams:
On August 28, 2024, Division of Water Resources (DWR) Raleigh Regional Office (RRO) staff Jesse Barnes conducted
a routine compliance evaluation inspection of the single family wastewater treatment system located at 4901
Whitfield Rd, Durham, NC 27707 in reference to NPDES General Permit NCG550000 and Certificate of Coverage
(COC) NCG551737. Owner Kathleen Adams (yourself) was present during the inspection. A review of the subject
permit, operations, and maintenance was completed. This review reflected compliance with the subject permit.
Any determined permit violation(s) and item(s) of concern are listed below, along with other observations.
Inspection findings are detailed in the attached Water Compliance Inspection Report.
s) of Concern
1. The inspection and maintenance agreement with a contractor includes annual effluent sampling, but no
laboratory results were provided. Part I, Section C. (3.) of the subject permit requires the system effluent
to be sampled annually by a certified laboratory for multiple parameters (see permit) and such data must
be retained and available for review for at least three years.
2. Outfall 001 was not located during the inspection and the permittee was unaware of the outfall location.
Inspection and maintenance records by a contractor indicate the outfall has been inspected annually and no
visible solids or other obvious evidence of system malfunction were observed. Please locate and mark the
outfall for future DWR inspections.
Other Observations
The subject facility is a single family wastewater treatment system located at 4901 Whitfield Rd, Durham, NC 27707.
Version 2.0 of Certificate of Coverage NCG551737 for NPDES General Permit NCG550000 became effective June 23,
2021, and will expire at midnight on October 31, 2025. The subject facility is authorized to discharge wastewater
treated in accordance with the subject permit via outfall 001 to the nearby Church Branch (Stream Index: 16-41-1-
9), classified as Water Supply V (WS-V) and Nutrient Sensitive Waters (NSW) of the State within the Cape Fear
River Basin (Sub -basin 03-06-05).
The permittee maintains an inspection and maintenance contract with McFarland Septic. A copy of the contract was
provided and added to the facility's file. The system is inspected twice annually and maintained as needed. The
system consists of the following components: an Advantex pretreatment pod system, septic tank, processing tank,

UV disinfection, and outfall. According to provided records, all components were inspected most recently on May
24, 2024 and appeared to be operating properly. No obvious signs of system failure were observed during the
inspection.
Corrective actions that address Items of Concern 1 and 2 (above) should be planned and completed within a
reasonable time. Failure to complete corrective actions within a reasonable time may result in further
enforcement actions.
Please be aware that the Division may take enforcement action against any individual who fails to comply with the
requirements of all applicable permits, regulations, and/or statutes. Pursuant to NC General Statute (NCGS) 143-
215.6A, a civil penalty of not more than twenty-five thousand dollars ($25,000.00) may be assessed against any
person who violates or fails to act in accordance with the terms, conditions, or requirements of any permit, or
applicable regulations, issued pursuant to G.S. 143-215.1.
