Todd Robinson, David Rogers, and Matthew Hukill from EnivroLink, David Smith from the Cottages of Boone, and Jeremey Cumbo of Boone Owner, LLC, were present. A review of available records, treatment units, storage tanks, monitoring wells, and collection system was completed. This review reflected non-compliance with permit requirements. The following deficiencies were noted: 1. Standing water was observed next to the wet weather storage tank. Staff also noted this in correspondence dated January 9, 2023. Per Permit Condition V.1 the Permittee shall perform inspections and maintenance to ensure proper operation of the wastewater treatment and irrigation facilities and Permit Condition V.2 the Permittee shall inspect the wastewater treatment and irrigation facilities to prevent malfunctions, facility deterioration, and operator errors... Take measures to improve the stormwater diversions to prevent water from ponding around the storage tanks. To remove the floating mats from the wet weather storage tank, the tank was emptied and an access hatch cover was removed. For the wet weather storage tank to be put back in service the hatch cover and a new gasket must be installed. The same method will be used to remove the mats from the 5-day upset tank. Please provide an explanation for how the hatches will be leak tested prior to effluent entering the tanks again. B. Mr. Smith reported that an overflow from one of the grinder tanks within the collection system was noticed on August 26, 2024. An estimated 50 gallons left the tank and reached ground surface. Mr. Smith made necessary repairs and limed the impacted area. Mrs. Williams provided the 5-day report form to Mr. Robinson on August 30, 2024, and requested it be submitted by September 6, 2024. Per 15A NCAC 02T .0403A(a)(1) the collection system shall be effectively maintained and operated at all times to prevent discharge to land or surface waters and 15A NCAC 02T .0403(a)(7) overflows and bypasses shall be reported to the appropriate Division regional office... Moving forward, be sure to report all bypasses or overflows from the collection system and wastewater treatment facility within twenty-four (24) hours of first notice. Other observations: • No leaks were noted around the knife valve. There have been some issues with the air hose that controls the knife valve. • The outlet pipe on the EQ tank has been installed. The level sensor in the EQ tank was also replaced when the outlet pipe was installed. • Drip zones are still being repaired by EnviroLink. • All pumps in the WWTF were operational. • The bank above the storage tank had contractor matting placed on it. • The anoxic tanks were replaced on July 24th, 2024. • Leaks in storage tanks are still being repaired as needed. • The irrigation system was calibrated 12/29/2023. • The wet weather storage tank was emptied, and mats removed from the storage tank. The 5-day upset tank is currently being emptied by pulling effluent from the tank and being transferred to the large lift station to be retreated • While the 5-day storage tank is offline, pump and haul will be used to remove the effluent> 10NTU • Freeboard was reported as 24' for both tanks since March 2024. Mr. Robinson stated the tanks were being pulled down starting in March to begin replacing the anoxic tanks and remove the mats. • The brush screen room was in good condition. The full dumpster has been removed, and the dumpster is emptied 1x3months. • The out of order UV bank was repaired. This was listed in a deficiency in correspondence dated January 9, 2023. • The 2024 soil samples showed that all drip zones had a sodium index of about 3 and exchangeable sodium value of 30%. The 2023 soil samples show lower results. The higher values could be a result of sample contamination. Staff recommended washing all sample equipment prior to the next soil sample event. • The flow meter is an inline meter. It was calibrated 12/29/2023. • The turbidimeter was calibrated on 12/29/2023. An updated copy was provided on 8/29/2024. • Z3 had a high flow warning. Z4 had limited irrigation compared to the other zones, likely due to the fluctuation in incoming flow and current WWTF operations. • Mr. Robinson stated that pump and haul will be used if turbidity >10NTU while the 5-day upset tank is out of service. • A hard copy of the most recent permit was not available, but it was printed while staff were on site. • The large lift station had a float tangled around the pump stands. This was corrected while staff were on site. • COB staff inspect the grinder pump stations, and Envirol-ink staff inspect the two main lift stations. • An ORC designation form was submitted for the WW2 classification on 9/4/2024. Mr. Rogers was changed to the ORC. • The new anoxic tanks do not have a valve at the bottom and additional valves were added to isolate the tanks if needed. • Sludge is removed once every 2-3 months, with 2-3 loads removed at each event. • Staff observed Z4 irrigating while on site. No warnings or errors were noted and the irrigation pump was operational. Reason for Visit: Routine Yes No NA NE Aboveground storage tank 1562000 Gallons ■❑❑❑ NONE 2 Feet 24 Feet Page 9 of 9 15A NCAC 02T .0403 PERMITTING BY REGULATION (a) Collection systems having an actual, permitted or Division -approved average daily flow less than 200,000 gallons per day shall be deemed permitted, pursuant to Rule .0113 of this Subchapter if the system meets the criteria in Rule .0113 of this Subchapter and all criteria required in this Rule: (1) The collection system shall be effectively maintained and operated at all times to prevent discharge to land or surface waters and to prevent any contravention of groundwater standards or surface water standards. (2) A map of the collection system shall have been developed and shall be maintained. (3) An operation and maintenance plan, including pump station inspection frequency, preventative maintenance schedule, spare parts inventory, and overflow response shall have been developed and implemented. (4) Pump stations that are not connected to a telemetry system shall be inspected by the permittee or its representative every day, 365 days per year, unless the permittee demonstrates that daily inspections are not necessary because the pump station has sufficient storage capacity, above the elevation at which the pump activates, to justify a longer inspection interval. In no case shall the inspection interval exceed seven days. Pump stations that are connected to a telemetry system shall be inspected once per week. (5) High -priority sewers shall be inspected by the permittee or its representative once every six - months, and inspections shall be documented. (6) A general observation by the permittee or its representative of the entire collection system shall be conducted once per year. (7) Overflows and bypasses shall be reported to the appropriate Division regional office in accordance with 15A NCAC 02B .0506(a), and public notice shall be provided as required by G.S. 143- 215.1 C. (8) A Grease Control Program shall be in place as follows: (A) For publicly owned collection systems, the Grease Control Program shall include bi- annual distribution of educational materials for both commercial and residential users and the legal means to require grease interceptors for new construction and retrofit and if necessary, of grease interceptors at existing establishments. The plan shall also include legal means for inspections of the grease interceptors, enforcement for violators and the legal means to control grease entering the system from other public and private satellite collection systems. (B) For privately owned collection systems, the Grease Control Program shall include bi- annual distribution of grease education materials to users of the collection system by the permittee or its representative. (C) Grease education materials shall be distributed more often than required in Parts (A) and (B) of this Subparagraph if necessary to prevent grease -related sanitary sewer overflows. (9) Right-of-ways and easements shall be maintained in the full easement width for personnel and equipment accessibility. (10) Documentation of compliance with Subparagraphs (a)(1) through (a)(9) of this Rule shall be maintained by the collection system owner for three years with the exception of the map, which shall be maintained for the life of the system. (b) Private collection systems on a single property serving an industrial facility from which the domestic wastewater contribution is less than 200,000 gallons per day shall be deemed permitted. (c) The Director may determine that a collection system shall not be deemed to be permitted in accordance with this Rule and Rule .0113 of this Subchapter. This determination shall be made in accordance with Rule .0113(e) of this Subchapter.
