Foltz 670 Country Ln Winston-Salem, NC 27107 NORTH CAROLINA Environmental Quality August 19, 2024 SUBJECT: Compliance Evaluation Inspection 670 Country Ln, Winston-Salem, NC 27107 NPDES General Wastewater Discharge Permit #: NCG550000 Certificate of Coverage: NCG550529 Forsyth County On July 31st, 2024, Ron Boone, Brooke Stevens, and Michael Gearren, of the Winston- Salem Regional Office of the NC Division of Water Resources, conducted a compliance evaluation inspection of your home's discharging wastewater treatment system. You were not present for the inspection due to work commitments but spoke with Mr. Boone via phone prior to the inspection. Thank you for your assistance and cooperation. Attached to this letter is a Water Compliance Inspection Report that details the findings of the inspection. Violations noted during the inspection were: 1. Septic tank hasn't been pumped since about 2010 (as discussed pre -inspection on the phone); 2. Annual fees unpaid since 2017; and, 3. The effluent has not been tested (as discussed pre -inspection on the phone);. Other noted items of concern were: 1. The chlorinator and effluent pipe were accessible, but the chlorinator could not be opened to see if there was chlorine in it and the type of chlorine used could not be evaluated during the inspection. We have attached a copy of the NCG550000 general permit, a technical bulletin, a list of NC commercial certified labs that can test the effluent from your system as required by the permit, and information regarding the proper type of chlorine to use. Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) Dear Mr. Foltz, On July 31 st, 2024, Ron Boone, Brooke Stevens, and Michael Gearren, of the Winston-Salem Regional Office of the NC Division of Water Resources, conducted a compliance evaluation inspection of your home's discharging wastewater treatment system. You were not able to attend the inspection due to work obligations. We were able to locate the chlorinator and discharge pipe. No concerns were noted. Mr. Boone spoke with you by phone prior to the inspection and you informed him that the tank was last pumped in 2010 and that you check the chlorine once yearly. You also informed him that four people live in the home. Please be aware that you should have the septic tank pumped no less frequently than once every three to four years with four people living in the home. The permit actually requires you to have the solids level in the tank checked yearly and have it pumped whenever the solids level exceeds 1/3 of the liquid depth of the tank. Failing to pump the tank as required can result in severe damage to the treatment system, which in turn requires expensive repairs or greatly reduces the value of your home. Septic tank and filter failure may also result in severe property damage, public health hazards, and/or damage to the environment. The permit also requires you to check the chlorinator weekly and add more chlorine as required. Failure to do so may result in the discharge of large amounts of Fecal Coliform bacteria which can also cause severe environmental damage and/or death to wildlife in the receiving stream. You should also ensure you are using the proper chlorine. The proper chlorine contains approximately 70 percent calcium hypochlorite. Use of swimming pool type chlorine can cause unnecessary hazards and inefficient chlorination, as well as cost the user more money in the end. Furthermore, the permit requires you to have the system effluent tested once per year. Please have the system effluent tested by a NC certified laboratory as soon as practical. We have attached a list of certified laboratories that can properly test the effluent as required by the permit. We have also attached a copy of the general permit, technical bulletin, and chlorine information. Finally, the permit annual fee of $60 has not been paid since 2017. Please begin paying the annual fee when you receive the invoices each year. Failure to comply with the requirements of the permit subjects you to non-compliance notices and even civil penalty assessments.
