On October 27, 2011,1, Cheng Zhang of the Raleigh Regional Office conducted a compliance evaluation
inspection of the subject facility. The assistance of Mr. Timothy Aucoin, the operator in responsible charge
(ORC) of the facility, was appreciated as it facilitated the inspection process. The inspection report is attached.

The current permit expired on July 31,2001. A permit renewal application was received by the Division
on January 31. 2011, and is under review.
2. Under a Sanitary Sewer Service Agreement with the Orange Water and Sewer Authority (OWASA), the
facility discharges treated wastewater (permitted to be discharged through Outfall 001) into OWASA's
Mason Farms Wastewater Treatment Plant. The facility has not discharged through Outfall 001 since
April 2001.

The facility has developed and implemented a Stormwater Pollution Prevention Plan (SPPP). The plan
was reviewed during the inspection, which included all required components. Records of monitoring

The UNC Cogeneration Facility consists of: an oil/water separator, equalization basins, caustic rapid mix
tank, polymer feed system, two settling tanks, acid rapid mix tank, Parshall flume (Outfall 001), and a
stormwater detention pond on the west side of the site (Outfall 002) and a stormwater detention pond on the
east side of the site (Outfall 003). The facility is authorized to discharge from said treatment works into an
unnamed tributary to Morgan Creek, which is classified WS-IV NSW waterbody in the Cape Fear River
basin.

(both analytical and qualitative), site inspections, employee training, and annual plan reviews/updates
were available and well organized. Secondary containment for all outdoor fuel tanks was adequate.
Outfalls 002 and 003 were observed and in good condition.
