REQUEST FOR MORE INFORMATION
Dear Mr. Wortman:
On December 17, 2007, the Division of Water Quality (DWQ) received. your application dated
December 14, 2007, to fill or otherwise impact 0.023 acres of 404/wetland, 1991inear feet of
perennial stream, and 251inear feet of intermittent stream to construct the proposed residential
development: The DWQ has determined that your application was incomplete and/or provided
inaccurate information as discussed below. The DWQ will require additional information in
order to process your application to impact protected wetlands and/or streams on the subject
property. Therefore, unless we receive five copies of the additional information requested
below, we will have to move toward denial of your application as required by 15A NCAC 2H
.0506 and will place this project on hold as incomplete until we receive this additional
information. Please provide the following information so that we may continue to review your
project.
Additional Information Requested:
1. Please verify with the U.S. Army Corps of Engineers that this project will be permitted
under a Nationwide 39 permit, and not a Nationwide 29 permit.
2. Please provide a location map/most recent NRCS soil survey map/USGS 1:24,000
topographic map for the project.
3. Please re-submit your site plans on full plan sheets at a scale of no smaller than 1"=50'.
4. Please provide cross section details showing the provisions for aquatic life passage.

5. Please provide the location of any proposed stormwater management practices as
required by GC3705.
6. Please provide plans and profiles for all the stormwater management devices as required
by GC3705.
7. Please indicate all stormwater inlets and outfalls on the site plan.
8. Please provide an inventory of proposed project imperviousness area based on the
estimated built-out conditions. Please include all proposed building footprints, roads,
driveways, sidewalks, gravel-surfaced areas, amenity areas, etc.
9. Proposed bottomless culvert(s) on projects that require DWQ 401 Certification, and
Isolated Wetlands Permit or Buffer Approval must include submittal of a signed and
sealed (by a Professional Engineer or Professional Geologist) geotechnical investigation
of subsurface soils which directly complies with the DIVISION OFHIGHWAYS -
GUIDELINES FOR DRAINAGE STUDIES AND HYDRA ULIC DESIGN,

nes.pdf ), which states, "When a shallow (3-5 foot max. depth) non-erosive rock
foundation is found throughout the proposed site, the structure can be built on footings
without a bottom allowing retention of the natural channel bed." The Geotechnical Unit
must confirm the foundation acceptability prior to final selection of the "bottomless"
culvert." The non-erosive rock foundation is necessary to minimize stream disturbance
during construction, and to prevent scour and differential settlement. The culverts must
safely provide conveyance of the design storm event, must be economical to construct
and maintain, and should maintain low flow channel and promote retention of bed
material. There must be a minimum of 50 feet between borings and 50 blows in less than
6" penetration per ASTM.
10. Please indicate proposed lot layout as overlays on the site plan.
11. Please locate any planned sewer lines on the site plan.
12. Please indicate all stream impacts including all fill slopes, dissipaters, and bank
stabilization on the site plan.
13. Please indicate all wetland impacts including fill slopes on the site plan.
14. Please locate all isolated ornon-isolated wetlands, streams, and other waters of the State,
along with topographic contours, as overlays on the site plan.

15. 199 linear feet of compensatory stream mitigation is required for this project. Please
provide a compensatory mitigation plan. The plan must conform to the requirements of
15A NCAC 2H .0500 and must be appropriate to the type of impacts proposed.
16. This Office recommends utilization of RCP culverts rather than the proposed rock fords
for the crossings at Stream Impacts Nos. 1, 5, 6, and 7.
Please submit this information within 30 calendar days of the date of this letter. If we do not
receive this requested information within 30 calendar days of the date of this letter, your project
will be withdrawn and you will need to reapply with a new application and a new fee.
This letter only addresses the application review and does not authorize any impacts to wetlands,
waters or protected buffers. Please be aware that any impacts requested within your application
are not authorized (at this time) by the DWQ. Please call Ms. Cyndi Karoly or Mr. Ian
