Dear Mr. Strader:
Division staff has confirmed that the subject permit is no longer required. Therefore, in
accordance with your request, NPDES Permit NCO066010 is rescinded, effective immediately.
If in the future your company wishes to again discharge wastewater to the State's surface waters
from the former Williamsburg Elementary School location, you must first apply for and receive a
new NPDES permit. Discharge of wastewater without a valid NPDES permit will subject the
responsible party to a civil penalty of up to $25,000 per day.

This letter is to advise you that the Williamsburg Elementary School
located at 1404 Mizpah Church Road, Reidsville, NC was closed for
students and staff on May 30, 2004. Consequently, there has been no flow
since that time. Therefore, Rockingham County Schools will not be seeking
renewal of a wastewater permit for this site. The Board of Education may
consider disposing of this property at a later date in time.

Dear Mr. Holcomb:
Division personnel have reviewed and approved your application for renewal of the subject permit. Accordingly, we are
forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements of North Carolina General
Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency
dated May 9, 1994 (or as subsequently amended).
Please be aware that the Haw River is listed as an impaired stream on North Carolina's 303(d) list. This means that the
stream does not meet all water quality standards. The Division is working toward the improvement of water quality in the Haw
River. If the Division's current initiatives do not result in improved water quality, removal of discharges to the Haw River may
be necessary for the health of the stream. Although the Division believes multiple sources are the cause of degradation to the
Haw River, we will continue to monitor compliance at your facility. If noncompliance with your permit's effluent limits is shown
to be a direct cause of stream degradation, the Division will recommend removal of your facility's discharge.
Your facility discharges to a nutrient -sensitive water (NSW). In 1997 the Clean Water Responsibility Act (House Bill 515)
was enacted, with the intent to strengthen the control of nutrients from all discharges to NSW waters. Facilities permitted to
discharge greater than 500,000 gallons per day (and any new or expanding facilities) will be subject to nutrient limitations for total
nitrogen and total phosphorus according to House Bill 515 requirements. Since modifications to treatment systems are expensive
and time consuming, it is highly recommended that you begin planning for this possibility now if a future expansion is anticipated
at your facility.
If an), parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have
the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request must
be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the
Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is
made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require modification or
revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain other permits which may be
required by the Division of Water Quality or permits required by the Division of Land Resources, the Coastal Area Management
Act or any other Federal or Local governmental permit that may be required. If you have any questions concerning this permit,

Elementary School.. The Rockingham County School System is hereby requesting renewal of
permit # NCO036994 and permit #NC0066010. There have been no changes at these two
facilities since the issuance of the last permits. Also, enclosed are maps that we hope will be

SLUDGE MANAGEMENT PLAN:
These two plants do not have sludge removal. The sludge and solids generated are pumped by
Billingsley Septic Tank Company in the month of June each year after school closing for the
summer.

The Division is beginning the cycle of permit renewals for the Cape Fear river basin. An examination of the Cape
Fear basin plan and the NPDES permit schedule has revealed that the existing expiration dates in Cape Fear NPDES
permits do not coincide with the revised permit expiration dates for the basin. This discrepancy could create unnecessary
delays in the processing of over 200 permit renewals. Accordingly, the Division is changing the permit expiration dates for
NPDES permits in the Cape Fear river basin.
This permit modification changes the expiration date of the subject permit in subbasin 30601 to April 30, 2001.
Please find enclosed the revised permit cover page. Insert the new cover page into your permit and discard the old page.
All other terms and conditions contained in the original permit remain unchanged and in full effect. This permit
modification is issued under the requirements of North Carolina General Statutes 143-215.1 and the Memorandum of
Agreement between North Carolina and the U. S. Environmental Protection Agency.
In accordance with the modification of the permit expiration date, the new due date for your renewal application was
November 1, 2000. Renewal application forms are enclosed. If you need additional copies of the form, you may
download it from the NPDES web site at http://h2o.enr.state.nc.us/NPDES/documents.html. You will need to
download Short Form D. If you have already submitted a permit renewal form, there is no need to submit any other
forms to the Division; the application will be processed during the renewal period for your subbasin.
If any part of this permit modification are unacceptable to you, you have the right to an adjudicatory hearing upon
written request within thirty (30) days following receipt of this letter. This request must be a written petition conforming
to Chapter 150B of the North Carolina General Statutes, filed with the Office of Administrative Hearings (6714 Mail
Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.

Dear Mr. Hardy:
Chlorine, a widely used wastewater disinfectant for the treatment of coliform organisms, often
remains instream in residual amounts that may prove to be toxic under critical low stream flow
conditions. In the last decade, EPA assessed the potential adverse effects of chlorine to the aquatic
environment and has taken steps to reduce the impacts through the development of federal criteria.
In 1986, EPA recommended that all states have a chlorine standard by their next triennial review of
water quality standards.
In revising its water quality standards in 1989, North Carolina developed an action level for
chlorine of 17 ug/1(freshwater classes only). In addition, the fecal coliform limit was reduced from
1000 colonies/100 ml to 200 colonies/100 ml. Under a new DEM procedure, dechlorination and
chlorine limits are now recommended for all new or expanding dischargers proposing the use of
chlorine for effluent disinfection. The Division is reviewing chlorine levels from all existing
dischargers as part of their NPDES permit renewal process.
Our records indicate that chlorine from your facility's effluent discharge may be toxic to aquatic
life in the receiving stream under low flow conditions, i.e., the amount of chlorine discharged causes
an exceedance of the instream action level for chlorine (17 ug/l for chronic effects and 28 ug/l for
acute effects) under 7Q10 conditions (the lowest average daily flow for seven (7) consecutive days
during a ten (10) year period). Action should be taken to reduce the effluent concentration of
chlorine to an acceptable level. Based on your instream waste concentration of 100%, an acceptable
level of chlorine in your effluent is 17 ug/l or 0.017 mg/l. If this level is not feasible, you should
consider dechlorination or alternate methods of disinfection for your facility to ensure that both
chlorine and bacterial limits are met. In addition, if your facility plans to undertake any phase of
construction, dechlorination or alternate disinfection should be included. However, please note that
an authorization to construct must be obtained from this Division prior to any alteration to your
treatment plant.

The Division is currently reviewing its water quality regulations pertaining to chlorine. In the
future, effluent limits and/or dechlorination may be required of existing facilities with chlorine

The current enrollment at Williamsburg Elementary is
approximately 249 students and teachers. Although the
sandfilter system appears to be well maintained, the
facility has consistently violated the BOD and NH -N
limitations. Please note that the discharge is into a
designated zero flow stream.

Dear Mr. Kinlav:
A letter of request for an Authorization to Construct was received
October 29, 1986 by the Division and final plans and specifications for the subject
project have been reviewed and found to be satisfactory. Authorization is hereby
granted for the construction of a chlorinator with a contact tank, and step
reaeration system in accordance with the Standard drawings for North Carolina
Schools.
This Authorization to Construct is issued in accordance with Part
III paragraph C of the NPDES Permit No. NC0066010 issued October 1, 1986, and
Shall be subject to revocation unless the wastewater treatment facilities are
constructed in accordance with the conditions and limitations specified in Permit No.
NC0066010.
The Permittee must employ a certified wastewater operator in
accordance with Part III, paragraph D of the reference permit.
The sludge generated from these treatment facilities must be disposed
of in accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
In event the facilities fail to perform satisfactorily in meeting its
N'PDES permit effluent limits, Reidsville City Schools shall take such immediate
corrective action as may be required by this Division, including the construction of
additional wastewater treatment and disposal facilities.
