Regional Office. It should have the project name, project number, and the close out termination
language in the comments section.
On the NOT Certification Form, the Legally Responsible Person should be Derek Knight. In order for Clay
Worthy to sign the NOT Certification form and submit the eNOT, Clay Worthy needs to print their name
and title on the "Print Name and Title of Signed if Authorized" line of the NOT Certification Form.
