Subject: Minor NPDES Permit Modification
Permit NCO087840
Middlesex WTP
Nash County
Dear Ms. Bissette:
Division personnel have reviewed and approved your request for modification of the above referenced
permit. Based on the proximity of the discharge to the receiving stream, the batch nature of the discharge,
and the minimal volume of discharge, the instream monitoring requirement has been deleted. Furthermore,
installation of an additional holding tank may not be necessary. Any construction deemed necessary by the
Town to meet effluent limitations contained herein must be approved by the Division of Water Quality's
Construction Grants & Loans program. This permit modification is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and
the U.S. Environmental Protection Agency dated May 9, 1994 (or as subsequently amended).
Please take notice that this permit is not transferable. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Water Quality or permits
required by the Division of Land Resources, Coastal Area Management Act, or any other Federal or Local
governmental permits which may be required.
If you have any questions or need additional information, please do not hesitate to contact Mark
McIntire of my staff at (919) 733-5083, extension 508.
Sincerely,
zan Klimek, P.E.
