Directions to Site: Take NC 29 N Business from Reidsville. Right on Narrow Gage Rd. Rt. on Pennington Rd. Rt. on Vivian. Next to last house on Rt. in cul-de-sac. 6. Discharge Point- Latitude: 36° 22' 52" Longitude: 79° 38' 31" Attach a USGS Map Extract and indicate treatment plant site and discharge point on map. Describe receiving stream features and pertinent downstream uses: Creek flows through suburban woodlands. PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1. a. Please provide a description of existing or substantially constructed wastewater treatment facilities: Existing: 1000 gallon ST w/396 sq. ft. primary sandfilter and 198 sq. ft. secondary sandfilter, tablet chlorinator, combined discharge line with neighbor, and step aeration unit (poor). Discharge, if any, flows overland about 100' to very small creek. f. Please provide a description of the proposed wastewater treatment facilities: g. Possible toxic impacts to surface waters: Residual chlorine. h. Pretreatment Program (POTWs only) in development approved should be required XXX not needed 2. Spray Irrigation: no Connection to Regional Sewer System: The economic alternatives analysis should be required of this SFR prior to the next renewal as the City of Reidsville will probably extend sewer service near this site when Zarn, Inc. is annexed. Other Special Items: The owner should be advised that the chlorinator must be accessible for inspections and easily located by installing a box w/ a cover lid. Currently, the tubes are cut off at ground level and do not appear to have been routinely used (no wear marks). PART IV - EVALUATION AND RECOMMENDATIONS WSRO recommends the permit be reissued contingent on an economic alternative analysis being required for subsequent renewals.
