Gouge Residence
Wastewater Facilities
Rockingham County
Dear Mr. Gouge:
The final plans and specifications for the subject project have been reviewed
and found to be satisfactory. Authorization is hereby granted for the construction
of a 600 GPD wastewater treatment facility consisting of a 1,000 gallon septic tank,
a 522 square foot subsurface aand filter, tablet chlorinator, chlorine contact cham-
ber, cascade aerator, and all associated piping and appurtenances to serve the Gouge
Residence.
This Authorization to Construct is issued in accordance with Part III paragraph
C of NPDES Permit No. NC0.055671 issued April 29, 1983, and shall be subject to revo-
cation unless the wastewater treatment facilities are constructed in accordance with
the conditions and limitations specified in Permit No. NC0055671.
The sludge generated from these treatment facilities must be disposed of iu
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
The Winston Salem Regional Office, telephone number 919/761-2351 shall be noti-
fied at least twenty-four (24) hours in advance of backfilling of the installed sub-
surface filter system so that an in-place inspection can be made of said system prior
to backfilling. Such notifications to the Regional Supervisor shall be made during
the normal office hours from 8:30 A.M. until 5:30 P.M. on Mnnday through Friday,
excluding State Holidas.
