Costner Residence
Wastewater Facilities
Rockingham County
Dear Mr. Costner:
The final plans and specifications for the subject project have been
reviewed and found to be satisfactory. Authorization is hereby granted
for the construction of a 300 GPD wastewater treatment system consisting
of a 1000 gallon capacity septic tank, a 264 square ft. subsurface sand
filter, a tablet chlorinator, baffled chlorine contact chamber a approx-
imately 150 lineal feet of 4-inch gravity effluent line to serve the
Costner Residence.
This Authorization to Construct is issued in accordance with Part III
paragraph C of NPDES Permit No. NC0055654 issued March 18, 1983, and shall
be subject to revocation unless the wastewater treatment facilities are
constructed in accordance with the conditions and limitations specified
in Permit No. NC0055654.
The sludge generated from these treatment facilities must be disposed
of in accordance with G.S . 143-215 . 1 and in a manner approvable by the
North Carolina Division of Environmental Management. The Winston-Salem
Regional Office, Phone No. 919/761-2351 shall be notified at least twenty-
four (24) hours in advance of backfilling of the installed subsurface filter
trench so that an in-place inspection can be made of said system prior to
backfilling. Such notification to the Regional Supervisor shall be made
during the normal office hours from 8:30 A.M. until 5 :30 P.M. on Monday
through Friday, excluding State Holidays.
