On July 29, 2024, Jim Westcott from the Raleigh Regional Office visited your Single-family
residence wastewater treatment and spray irrigation system to evaluate compliance with the
subject permit. The assistance of Mrs. Breeze during the inspection was appreciated.
The facility is permitted for the continued operation of a construction and operation of a 480
gallon per day (GPD) single-family residence wastewater treatment and irrigation facility
consisting of. a 1,500 gallon baffled septic tank with an effluent filter; an 800 gallon Advantex
Treatment Unit (AX20-RT) with a 16.75 gallon per minute (GPM) recirculation pump; a 6 GPM
3 -inch diameter ultraviolet (UV) disinfection unit; a 3,500 gallon pump/storage tank with an
18 GPM dosing pump; a 0.27 acre drip irrigation field with 0.61 gallon per hour (GPH) emitters
on 2 foot spacings and 3 foot laterals; a rain sensor; and all associated piping, valves, controls,
and appurtenances, with no discharge of wastes to the surface waters.
The permit, which was issued May 11, 2018, and expired April 30, 2023. Division records
indicate the required annual fee for the period ending April 30, 2025, due May 11, 2024, has
been paid.
Findings during the inspection were as follows:
1. Treatment system Operation: The wastewater treatment system shall be effectively
maintained and always operated so there is no discharge to surface waters, nor any
contravention of groundwater or surface water standards. The treatment system components
are to be inspected weekly and monthly by the permittee.
At the time of inspection, the permittee was on -site. A comprehensive evaluation of the
facility system components were tested and evaluated at the time of inspection.
2. Pumping the septic tank: You are required to inspect the septic tank annually to
determine if solids must be removed or if other maintenance is necessary. Septic tanks
should be pumped out every five years or when the solids level is found to be more than 1/3 of
the liquid depth in the septic tank compartment, whichever is greater. A pumping company can

check the status periodically and determine when pumping is required. The non -discharge
permit requires the permittee to retain records associated with residuals disposal for a period
of 5 years. The permittee did not provide a receipt of the date the residuals were last
removed from the system.
Within 30 days of receipt of this letter, please provide a contract receipt of the volume
of residuals removed from the facility.
3. Disinfection: Your system uses an ultra -violet (UV) light system for disinfection of the
discharged wastewater. A UV disinfection system provides effective disinfection and
prevents/limits harmful bacteria from discharging to the environment. Unless the UV
disinfection unit is monitored by telemetry, the unit shall be checked weekly. The lamp(s) and
quartz sleeve(s) shall be cleaned or replaced as needed to ensure proper disinfection of the
discharged wastewater. The CLEARSTREAM UV filter housing enclosed with the lamp was
evaluated for proper operation at the time of inspection. The control junction box with
rain sensor, audible/visual high-level alarm was located and tested.
4. Proper Operation and Maintenance: The system shall be inspected monthly to make
certain of the proper operation of the spray and drip irrigation system and that the soil is
assimilating the disposed treated wastewater with no surface runoff, and that no objectionable
odors are being generated. Any visible signs of a malfunctioning system shall be documented,
and steps taken to correct the problem. At the time of inspection, the drip irrigation
system was not evaluated for operation. The field vegetative cover was in good
condition, although overgrown. There appeared to be no evidence of ponding. No fence
was installed surrounding the drip field. A UNITRONICS JAZZ PLC Automatic Controller
was operational at the time of inspection and all four zones and filter backflush were
in auto mode.
Within 30 days of receipt of this letter, please provide a photo documenting the
required fence installation surrounding the drip irrigation area.
Compliance Issues
The following issues are not compliant with the permit:
1. Permit Condition III 13A states the conditions of the Operation and Maintenance
Requirements "The septic tank shall be checked annually and pumped out as needed.
The septic tank effluent filter shall be checked and cleaned at the same frequency as the
septic tank. " [15A NCAC 18A .1900, 02T. 0605]
At the time of inspection, the septic tank was found to be completely full and is
in immediate need of the residuals removed from the system.
2. Permit Condition I11.8 states "Public access to the irrigation sites and wastewater
treatment facilities shall be prohibited. Fencing shall be provided to prevent access to the
irrigation site (minimum 2-strand wire) and treatment units shall be secured with locks on
all tank accesses and control panels" [15A NCAC 02T .0605(I)].
At the time of the inspection, there was no fencing in place surrounding the drip
field. As indicated, a minimum of 2-wire fencing is required around the entire
drip field.

Within 30 days of receiving this letter, please submit a written response to the Raleigh
Regional Office indicating the actions you will, or have taken to address the compliance
issues listed above.
Please continue to periodically inspect and evaluate the wastewater spray irrigation system
components are properly maintained and operating as designed. Maintain all associated
operational records onsite for a period of five years as required by the subject permit.
