Dear Ms. Eplin:
Dewberry Engineers Inc. has modified the Erosion & Sediment Control Plan for this project to
accommodate for changes in the overhead distribution line layout and an addition of 0.3 acres of land
disturbance. This resulted in the limits of disturbance increasing from 10.0 acres to 10.3 acres. We are
requesting a permit modification to include this adjustment for construction purposes.
Included in this submittal are the following:
• $100 supplemental application fee (Check No.201461)
• One (1) copy of the revised Erosion Control Narrative.
• One (1)original and one (1) copy of the revised completed and notarized Financial
Responsibility/Ownership Form and deeds.
• One (1) set of Revised Drawings (full size).
• One (1) copy of the original letter of approval.
