be emailed or hand-delivered only. Illegible faxed maps will not be accepted.
All proposed infiltration areas and existing, active utility lines located within the proposed
basin/trench must be marked and flagged. If these areas are not flagged, the Soils Scientist
reserves the right to decline to do the investigation. If the proposed infiltration system will be
located in an area of existing pavement and there is no open area nearby, equipment capable of
breaking through the impervious layer must be provided. The soils investigation does not take
the place of a soils report prepared by an appropriate professional. The Soils Scientist will only
verify the soil conditions that are reported in the Soils Report, and make a determination as to
the suitability of the site to meet the infiltration design requirements under NCAC 2H.1000, and
assumes no liability should the system fail.

Dear Mr.Avolis,
ECS Carolinas, LLP (ECS) recently conducted an infiltration evaluation for the proposed
infiltration area at the above referenced site located off of Tenth Street and Bridges Street in
Morehead City,Carteret County,North Carolina. This letter,with attachments, is the report of
our evaluation.
Field Testing
On September 9,2008 ECS met with Mr.Kevin Avolis with Avolis Engineering on site in order
to provide the requested test location. BCS then conducted an exploration of the subsurface soil
and groundwater conditions at one requested location shown on the attached Site Diagram. The
purpose of this exploration was to obtain subsurface information of the hi-place soils for the
proposed infiltration area. ECS explored the subsurface soil and groundwater conditions by
advancing one baud auger boring into the existing ground surface at the requested boring
location. ECS visually classified the subsurface soils and obtained representative samples of
each soil type encountered. ECS also recorded the groundwater and seasonal high water level
(SHWL) observed at the time of the hand auger baring. The attached Infiltration Evaluation
Form provides a summary of the subsurface conditions encountered at the hand auger boring
location.
The groundwater and SHWL were estimated at the boring location below the existing grade
elevation. Below is a summary of the boring location. The surface elevation was provided by
Avolis Engineering.

ECS has conducted an infiltration test utilizing a compact constant head permeameter near the
hand auger boring to estimate the infiltration rate for the subsurface soils. Infiltration tests are
typically conducted at two feet above the SHWL.

Dear Mr. Martin:
Your recent State Stormwater Management Application and check#12017 for the $505.00 application
fee are being returned because the application lacked the necessary basic elements to accept it for
review. The plans and any other documentation that were submitted have been discarded or recycled,
as appropriate. Please address the following noted deficiencies and provide the requested information:
® According to Kevin Avolis with Avolis Engineering, PA, Vincent Lewis, a soil scientist with
DENR, has not yet conducted an infiltration site visit, which is required for all infiltration projects
proposed in counties served by the Wilmington Regional Office. Please submit the Infiltration
System Investigation request form to Mr. Lewis in order to schedule a site visit.
® Please verify whether or not this site already has a permit. According to the narrative included
with the application package, this site had been previously develop and utilized as the
Morehead City Elementary School. According to the DWQ permit database, Morehead City
Elementary School has an active low-density stormwater permit, SW8 921104.
® Evaluate the need for additional inlets into the infiltration basin. Each inlet should handle a
maximum volume of 2 acre-inch.
® The wetlands note on plan sheet C1 indicates that no Section 404 wetlands are located within
the project limits. Are there non-Section 404 wetlands present? If so, please delineate.
Please resubmit the permit application package to the address below. Note that the 90-day statutory
permit review time begins upon receipt of a complete application. If you have any questions, please do
