Mr. Eplin, Thanks for the email today, please see the information you requested below. I have copied the Plant Manager and several members of the Environmental Team. Permit No: NCG070000 Start Date: 7/29/2024 Start Time: 11:00 am End Date: 8/2/2024 End Time: 3:00 pm Approximate Volume Released: 5000 gallons Summary: One of the heavy equipment operators was routinely cleaning the filter screen at the retention pond that leads to Outfall#2. The operator was using the excavator bucket to complete this task. While removing the screen the operator lost their grip of the filter screen and it fell puncturing the plastic pipe leading into the outfall#2. This puncture allowed water from the pond to free flow through the pipe and into outfall#2. The water that was flowing out of outfall#2 was coming directly out of the retention pond. He noticed the flow of water increasing and observed as the water flowed through the punctured plastic pipe and under the dewatering structure. He immediately notified his supervisor of the situation, and his supervisor generated a plan to rectify the issue. Though water quality was not compromised, the water did flow out of the pond at a faster rate than it was designed for, so we wanted to follow our permit instructions and draft this report of the upset to the NCDEQ. Less than 3 hours after the upset a plan had been generated and was initiated. The contractors we sourced for repairs are completing their finishing touches now. Before removing the punctured pipe, the contractors placed a structure in between the pond and the outfall to hold the water in the retention pond. They removed the punctured plastic pipe, set a foundation for the new plastic pipe, and secured the pipe with concrete to keep it in place. Multiple measures were put in place to prevent a reoccurrence of this upset. The first measure was to draft an operating procedure for completing such task in the future. The second measure was holding a meeting with all operators to go over the operating procedures for a clear understanding of how this task is to be completed in the future. Both have been completed. Good afternoon Jerry, I am contactingyou on behalf of the Gold Bond Building Products Mount Holly Plant(NCG070175)to report an upset within our stormwater system that has resulted in an unanticipated bypass of a stormwater control measure.Yesterday while routinely cleaningthe filter screen in our#2 retention pond, personnel from the plant lost their grip on the filter screen and it fell puncturingthe plastic pipe leading into the outfall#2.This puncture has allowed water from the pond to free flow through the pipe and into outfall#2.The water that is flowing out of outfall#2 is coming directly out of the retention pond and water quality is not a concern, but the water is flowing out of the pond at a faster rate than it was designed for so we wanted to report the upset to the NCDEQ.
