Haddock #998163 Dear Peggy Haddock, You are hereby requested to attend a Show Cause Meeting to be held at: Washington Regional Office on September 24, 2024 at 10:30am. Show Cause Meetings are conducted to allow a certified operator the chance to present supporting information why disciplinary action should not be taken, based on facts gathered by Regional Office staff. This meeting is a result of: The discharge of wastewater and improper waste application occurring at the Keene Farm (AWS740022) discovered on August 11, 2024. The excessive ponding of wastewater. The non -compliant lagoon levels and failure to notify the Division at the Keene Farm and Haddock Farm (AWS740101). Grounds for disciplinary action are set forth in 15A NCAC 08G .0802 (Disciplinary Actions) and § 90A-41 (Revocation of certificate). These actions include: 1. Engaged in fraud or deceit in obtaining certification. 2. Failed to exercise reasonable care, judgment, or use of the operator's knowledge and ability in the performance of the duties of an operator in charge. 3. Is incompetent or otherwise unable to properly perform the duties of an operator in charge. Please bring all pertinent information, such as operator logs, maintenance records, field notes, or letters to the owner that demonstrate you have, at a minimum, met the responsibilities of a certified operator as described in the Rules.
