Dear Mr. Ference:
Division of Water Resources' Central and Regional staff have reviewed the application package
received on May 30, 2024. However, the Division requires additional information before completing our
review. Please address the items on the attached pages no later than the close of business on September 16,
2024.
Please be aware that the Applicant is responsible for meeting all requirements set forth in North
Carolina rules and regulations. The Applicant is also responsible for any oversights that occur during the
review of the subject application package. The Division may return the application as incomplete pursuant
to 15A NCAC 02T .0107(e)(2) if any omissions are made when responding to the outstanding items in
Sections A through R or the Applicant fails to provide the additional information on or before the above -
requested date.
Please reference the subject application number when providing the requested information. The
Applicant shall sign, seal, and date (where applicable) all revised and/or additional documentation and
submit an electronic response to my attention via the Non -Discharge online portal.

A. Cover Letter:
1. The cover letter mentions the activation of existing monitoring wells MW-8 through MW-13 at this
site, but these monitoring wells do not exist in our database. Please provide the Well Construction
Record (FORM: GW-1) for each of these wells.
B. Application Fee:
1. A fee was enclosed with the application, which the cover letter states is to cover the application
fee. However, fees are only required for permit modifications that increase the generating facility's
residuals dry tonnage, add additional land application areas not previously approved, add additional
residuals sources, or adds new treatment/storage units. Renewals do not require an application fee
unless they include one of the above modifications, which does not appear to be the case for this
application. Please clarify the purpose of the fee.

Guidance for Residuals Land Application Permits" guidance document. Updated site maps for all
land application sites shall be provided. These maps shall provide the locations of monitoring wells
MW-8 through MW-13.
