Dear Mr. McMillan,
As we discussed on the phone this morning, enclosed are copies of my application to extend the
rip rap along the shore line of my home on Hyco Lake in Person County. Most of my shore line
already has rip rap (applied more than 35 years ago). However, a short length of about 50 feet
has no rip rap, and over the years, this part of the shore line has eroded badly. I am requesting
that you allow me to add rip rap to this short section. The project will involve laying a band of
rip rap along'the water line with little or no impact on the property above or below the water line.
As we discussed by phone, I will e-mail detailed pictures of the eroding shore line this coming
weekend. Please let me know if I can provide any additional information.

The following is an abbreviated application form for those applying fora 401 Water Quality
Certification to impact FERC (Federal Energy Regulatory Commission) regulated lake shore or
bottom under the U.S. Army Corps of Engineers Permit GP30. This Application cannot be used
for wetland or stream impacts. Please provide seven (7) copies of this application and supporting
information as well as anon-refundable application fee to the Division of Water Quality of $240.00
for impacts of to lake bottom of less than 1 acre and $570.00 for impacts to lake bottom of greater
or equal to 1 acre.
