Sullins Dee Freeman Governor Director Secretary September 1, 2011 Dennis & Lisa Sykes 5650 Lakeside Drive Pfafftown, North Carolina 27040 Subject: General Wastewater Permit NCG550209—Single Family Residence 5650 Lakeside Drive, Pfafftown, North Carolina Forsyth County Dear Mr. & Mrs. Sykes: Jenny Graznak of the Winston-Salem Regional Office performed a Compliance Evaluation Inspection on the sandfilter wastewater treatment system at your property located at 5650 Lakeside Drive in Pfafftown, Forsyth County, North Carolina on August 26, 2011. This type of inspection consists of two basic parts: a review of self-monitoring data, and an on-site inspection of the facility and its discharge. No one was home during the inspection, but Ms. Graznak spoke to you afterward via telephone. You hold a General Permit to discharge Domestic Wastewaters from Single Family Residences under the National Pollutant Discharge Elimination System (NPDES). According to our files, the system has a 750-gallon septic tank, a 261 square feet subsurface sand filter, and a chlorinator. The chlorination unit could not be located and you were unsure of its location. Therefore, the next time you have the septic tank pumped out, you should ask them to help you find the chlorination unit. The septic tank was last pumped out in 2010. The effluent from the system discharges to an unnamed tributary to Mill Creek in the Yadkin River Basin. The discharge pipe was not located during the inspection due to the thick summer vegetation and steep hill. The receiving stream appeared clear with no traceable solids, and no odor was noted on the day of the inspection. You stated that you have never seen any discharge from the pipe. If you do detect a discharge from your system, please check the permit text Part I Section A (attached to this correspondence), which specifies the limits and monitoring requirements for your permit. A Laboratory with North Carolina Wastewater Certification (a list of nearby approved laboratories is attached) must perform the monitoring.
