Single Family Wastewater Treatment Facility

Dear Mr. Fulcher:
The final plans and specifications for the subject project have been reviewed
and found to be satisfactory. Authorization is hereby granted for the construction
of a 450 GPD wastewater treatment facility consisting of a 1,000 gallon capacity
baffled septic tank, a distribution box, a 396 S.F. subsurface sand filter, a 198
S.F. polishing subsurface sand filter, a chlorine contact tank with tablet chlori-
nator, a 5 step cascade aerator, and associated 4-inch gravity piping and appur-
tenances
to serve the Paul
us A. Fulcher resi
dence in Rockingham County.
This Authorization to Construct is issued in accordance with Part III para-
graph C of NPDES Permit No. NC0055263 issued April 20, 1c83, and shall be subject
to revocation unless the wastewater treatment facilities are constructed in accor-
dance with the conditions and limitations specified in Permit No. NC0055263.
• The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
The Winston Salem Regional Office, telephone No. 919/761-2351 shall be noti-
fied at least twenty-four (24) hours in advance of backfilling of the installed
subsurface filter system so that an in-place inspection can be made of said system
prior to backfilling. Such notification to the Regional Supervisor shall be made
during the normal office hours from 8:30 A.M. until 5:30 P.M. on Monday through
Friday, excluding State Holidays.
