Dear Mr. and Mrs. Tilley:
On November 4, 2014, Jenny Graznak of this office performed a Compliance Evaluation Inspection
on the General Permit to discharge domestic wastewater from the single family residence located at
381 Osley Road in Madison, NC. According to county records, you are the current owner of this
property. You were present during the inspection.
A file review revealed that this permit is still listed under the previous owner's name: Tony Williams.
Due to non-payment and the non-receipt of a renewal application, this permit expired on July 31,
2013 and must be renewed as soon as possible. You will need to complete and return a Permit
Name/Ownership Change Request, which can be found at the following web address:

The inspector observed two chlorinator tubes, which did not have chlorine tablets in them. Please be
. aware that the Permit requires that the chlorinator be adequately supplied with chlorine tablets for
proper operation. Your septic tank hauler can help you purchase the correct tablets for your system.
One of the tubes showed signs of damage, which you stated you would handle.
The Permit also requires that the septic tank contents be pumped out on a regular basis to prevent
solids from clogging the sandfilter system. You stated that the tank has been pumped out twice since
you purchased the residence in December 2011. Ms. Graznak was unable to locate the discharge
pipe during her inspection. Please continue to search for it. Your septic tank hauler may be able to
assist with that as well.
