Dear Mr. Armstrong:
Revised engineering plans and a letter of request for amendment of Authoriza-
tion to Construct No. NC0052167 was received April 18, 1983, by the Division. The
plans have been reviewed and found to be satisfactory.
Authorization to Construct No. NC0052167, issued March 31, 1983, is hereby
amended to revise and relocate the subject wastewater treatment facilities on the
site and to increase the size of the subsurface filter trench from 391 to 416 square
feet.
This amendment does not alter the conditions, limitations, or the expiration
date of the permit or authorization.

Armstrong Single Family Residence
Wastewater Treatment Facilities

Dear Mr. Armstrong:
The final plans and specifications for the subject project have been reviewed
and found to be satisfactory. Authorization is hereby granted for the construction
of a 450 GPD wastewater treatment facility consisting of approximately 95 lineal
feet of 4-inch gravity piping, a 1,000 gallon septic tank, a distribution box,
391 square feet of subsurface filter trench, a chlorine contact tank with tablet
chlorinator, and associated appurtenances to serve the J. Wilbur Armstrong resi-
dence in Alexander County.
This Authorization to Construct is issued in accordance with Part III para-
graph C of NPDES Permit No. N00052167 issued March 29, 1983, and shall be subject
to revocation unless the wastewater treatment facilities are constructed in accor-
dance with the conditions and limitations specified in Permit No. NC0052167.
The Mooresville Regional Office, Phone No. 704-663-1699 shall be notified
at least twenty-four (24) hours in advance of backfilling of the installed subsur-
face filter system so that an in-place inspection can be made of said system prior
to backfilling. Such notifications to the Regional Supervisor shall be made during
the normal office hours from 8:30 A.M. until 5:30 P.M. on Monday through Friday,
excluding State Holidays.
