I have listed my comment responses below: Hello Joshua and Riyad, After reviewing the Fast Track Sewer System Extension Application for Academy Point, Phase 10,additional information is required in order to further process the application. Please provide the following information by 07/19/2024, and note that the requested information must be submitted within 30 days of this request, or the applicant will be required to submit a new application and application fee in accordance with 15A NCAC 02T .0107. 1. Is this an ARPA funded project? If so,_please include the DWI assigned ARPA project number in your response. No. 2. Section 11.1, X.2, and X.3 of the FTA, as well as the FTSE project name and downstream facility name, indicate 'point'. Your large USGS quad map also lists the project name as 'Academy Point, Phase 10'. However, in the letter of transmittal, it is written as 'pointe' and 'point'. The Town of Clayton's wastewater allocation request mentions the project name as 'Academy Pointe Ph 10', and your construction release map states 'Academy Pointe Phase 10'. Please ensure consistency in the name across all documents and resubmit electronically. Note: the division does not require construction drawings unless mandated by MDC and 15A NCAC 02T. All project names have been corrected to include the correct spelling "Pointe". 3. Section 11.4 of the FTA coordinates states that the project is within Neuse River Basin; however, section IX.4 was checked 'no'. Please provide the correct answer. Neuse River was provided in section IX.4 with the yes box checked. 4. Section IV of the FTA, Permit Number NCO024533 does not exist. Please provide the correct WWTF on the FTA and FTSE. Note that FTSE 10-23 is currently available and the most recent flow tracking form. @Chris Rowland @Joshua Baird have informed me that NCO024533 does infact exist and was formerly known as Little Creek WWTP, maybe that is where the confusion lies. 5. 'Wastewater Allocation Request' form was signed by Tim Summerville (JJARR, LLC); however, the approval form (page 4 of 6) missing Town Manager's/ Council's signature. Was this allocation approved? If so, please submit the approved letter. In addition, ass this flow allocation is different from the Session Law "Section 18.1.(e) of SL 2023-137, 75 GPD/BR rate shall apply to all applicable non- discharge permits issued on or after November 1, 2023", section V1.6 of the FTA should checked 'yes'. This box has been checked yes. Town of Clayton has accepted our design and construction documents. Allocation has been received, the formal request can be obtained from Town of Clayton if necessary. 6. This application is missing a clear USGS topo map and an aerial map. According to Section F of the Application Instructions, an 8.5-inch x 11-inch color copy of a USGS Topographic Map and an aerial map should be included with the application. The maps need to clearly identify the project area, including the closest surface waters, general location of the gravity sewer, pumpstations, and force mains, and the downstream connection points for the receiving sewer. Please provide the required maps according to the application requirements. Maps have been modified to be a larger scale for clarity. 7. Although you checked section IX.7 as 'no', without an aerial and a clear USGS map, the division was unable to determine if there were any aerial crossing/high priority sewer lines proposed for this project. Maps have been modified to be a larger scale for clarity. 8. Your narrative is inadequate. Please provide a more descriptive narrative with the requested flow and associated infrastructure. Size, length and material has been added to the narrative. 15A NCAC 02T .0107 Staff Review and Permit Preparation states: • "(2) (e) If an application is accepted and later found to be incomplete, the applicant shall be advised how the application or accompanying supporting information may be modified to make it complete.
