Subject: Narrative -Eastfield Pump Station 4
Town of Selma Wastewater Collection System
The Fast Track Sewer Application is being submitted for the proposed Eastfield Pump Station 4
on behalf of AdVenture Development. It will be located near the intersection of Providence
Boulevard and US Hwy 70 in the Town of Selma, Johnston County.
The following items are being submitted as part of this application:

The pump station will include approximately 1,232 linear feet of 8"PVC force main that
connects to the existing 16"Pine Level force main (permit number WQ0042434 operated by
Johnston County)under US Hwy 70, which then connects to the South Selma Regional Pump
Station (permit number WQ0043778). The gravity system feeding into Pump Station 4 was
permitted and approved through the project `Eastfield Crossings-Providence Boulevard',permit
number WQ0043120. Pump Station 4 is being permitted with zero flow since all upstream flows
are anticipated to be permitted within the next 10 years.
Please note that with the Fast Track Sewer System Extension Application, Section II-2, this is
labeled as a Proposed(New Permit)but technically it is replacing/upgrading the existing Pump
Station 4 that is currently in operation. The Town of Selma is unable to find the permit number
for the existing Pump Station 4.
Existing Pump Station 4 is currently receiving 15,000 GPD of effluent per the Town of Selma
and this flow will be diverted to the new Pump Station 4 one (1) day prior to existing Pump
Station 4 going offline. The Town of Selma is expected to provide supplemental historical flow
data regarding these flows and that information will be submitted once received.

I Chandra C. Farmer, P.E. certify to the best of my knowledge that the addition of
the volume of wastewater to be permitted in this project has been evaluated along the route to the receiving
wastewater treatment facility and that the flow from this project is not anticipated to cause any capacity
related sanitary sewer overflows or overburden any downstream pump station en route to the receiving
treatment plant under normal circumstances, given the implementation of the planned improvements
identified in the planning assessment where applicable. This analysis has been performed in accordance
with local established policies and procedures using the best available data. This certification applies to
those items listed above in Sections I and II plus all attached planning assessment addendums for which I
am the responsible party. Signature of this form certifies that the receiving collection system or treatment
works has adequate capacity to transport and treat the proposed new wastewater.

Hello Phillip and Charles,
After reviewing the Fast Track Sewer System Extension Application for Eastfield Pump Station 4, additional information is
required in order to further process the application. Please provide the following information by 7/16/2024, and note that the
requested information must be submitted within 30 days of this request,or the applicant will be required to submit a new
application and application fee in accordance with 15A NCAC 02T.0107.
1. Is this an ARPA funded project? If so,_please include the DWI assigned ARPA project number in your response.
2. FTA 06-21: (Please submit the latest FTA 10-23 with addressing the following)
a. Application No. WQ0044329 was returned on January 26, 2024 due to incomplete submission. Please keep the field
blank for the division.
b. Verify that the parcel ID in Section 11.5 is correct.
C. Verify that the Consultant Information in Section 111.1 and X.2 is correct and is the primary consultant for this
application.
d. Section IV.1 needs correction on the Facility Name as listed under the permit NCO030716
e. Section V1.5 should check if there is any pre-treatment is in effect for any industrial wastewater.
f. Section V1.6 needs to be checked.
g. Section V1.7 is missing information about the Establishment Type. Please refer to 02T.0114 to complete the table.
h. Section V1.7 states that the total flow to that Pump Station is 15,000 GPD. Please provide the permit number for this
existing flow. If the permit number does not exist, provide historical flow data, along with pump capacity and
specifications,to support the daily flow.This should be accompanied by the signature of an authorized
owner/person.
i. Section V111.5:The length of the force main does not align with the narrative.
j. Section IX.6 indicates compliance with additional permits/certifications.What are the other permits/certifications
that are dependent on this permit,or vice versa?
k. Although you checked 'no'for section IX.7, confirm that this project does not include any sewer collection lines that
are deemed "high-priority"?
I. Submit an updated FTA with recent signatures from the applicant and consultant.
3. Please submit an updated letter of transmittal. Note that all applications should be submitted properly as'new
submissions'. Previously returned application materials will not be accepted as supplements. Only refer to an existing
issued permit for modification application.
4. Your narrative needs correction following the instruction form FTA 10-23 section B. Please update and submit with a
recent signature.
5. FTSE 10-18: (Please submit the latest USE 10-23 with addressing the following)
a. Section I.g is incorrect. Please update as per the instruction form.
b. Downstream sewer permit number WQ0044329 was returned on January 26, 2024. Provide a valid downstream
sewer permit number.
c. Submit an updated USE with most recent signature.
6. Your maps still need proper identification of the pump station, direction of force main,and downstream sewer connection
point.According to Section F of the Application Instructions,an 8.5-inch x 11-inch color copy of a USGS Topographic Map
and an aerial map should be included with the application. The maps need to clearly identify the project area, including
the closest surface waters,general location of the gravity sewer, pumpstations,and force mains, and the downstream
connection points for the receiving sewer. Please provide the required maps according to the application requirements.
15A NCAC 02T.0107 Staff Review and Permit Preparation states:
• "(2) (e) If an application is accepted and later found to be incomplete,the applicant shall be advised how the application
or accompanying supporting information may be modified to make it complete.The staff shall advise the applicant:
(2) if all required information is not submitted within 30 days,the project will be returned as incomplete.Any resubmittal of
a returned application shall be accompanied with a new application fee."
