Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) On July 11th, 2024, Ron Boone and Michael Gearren, of the Winston-Salem Regional Office of the NC Division of Water Resources, conducted a compliance evaluation inspection at the Slate Sand Mining, LLC, Pinnacle, NC, sand dredging facility. This facility discharges to the Yadkin River under NC general NPDES discharge permit NCG520000, certificate of coverage NCG520061. The assistance and cooperation of Todd Tilley and Jody Phillips was greatly appreciated. The facility was very clean and well maintained. Mr. Tilley stated that the signage required in Section A of the permit has been posted on the river as required to warn canoers and kayakers of the presence of the sand dredging operation. The permit requires that the permittee test the effluent for turbidity twice annually. This testing must be done by a lab certified by the State of NC. Mr. Tilley was able to provide records of the required testing over the past three years. The testing is being done by the City of Mount Airy Water Treatment Plant. Mr. Boone noted no deficiencies or violations. Please send Mr. Boone photographic evidence of the signs required in Section A of the permit. Please also ensure it is discernable in the photos the side of the river that each sign is on.
