Division personnel have reviewed and approved your application for renewal of the subject permit. Accordingly,
we are forwarding the attached NPDES discharge permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the
U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently amended).
This final permit includes no significant changes from the draft permit sent to you on May 8, 2013.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you,
you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this
letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina
General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North
Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding.
Please note that this permit is not transferable except after notice to the Division. The Division may require
modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to
obtain other permits which may be required by the Division of Water Quality or permits required by the Division
of Land Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that
may be required.

SUPPLEMENT TO PERMIT COVER SHEET
All previous NPDES Permits issued to this facility, whether for operation or discharge are hereby
revoked. As of this permit issuance, any previously issued permit bearing this number is no longer
effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the
permit conditions, requirements, terms, and provisions included herein.

1. The grab sample for Oil and Grease should be skimmed from the surface in a quiescent zone, if
possible.
2. If Naphthalene is not detected above the method detection limit during the first eight sampling
events, its monitoring requirement shall be waived for the remainder of the permit term. If it is
detected above the detection limit during the first eight sampling events, the monitoring
requirement shall remain in effect for the remainder of the permit term.
There shall be no discharge of floating solids or visible foam in other than trace amounts.
A. (2.) . SITE MANAGEMENT PRACTICES AND SPILL RESPONSE
The Site Management Practices and Spill Response submittal of August 13, 2005 has been approved
and is an enforceable part of this permit.
FACT SHEET FOR EXPEDITED PERMIT RENEWALS
This form must be completed by Permit Writers for all expedited permits which do not require
full Fact Sheets. Expedited permits are generally simple 100% domestics (e.g., schools, mobile
home parks, etc) that can be administratively renewed with minor changes, but can include

This permit is for the discharge of petroleum contaminated stormwater originating at a gas station
pump island. The area is elevated and covered; the grates that receive the flow are also under the
covering. Flow to the system will only occur due to wind driven rainfall, so volume to the
treatment system is very small. The owner and its contract operations firm manage the situation
with weekly visits to the site, when grates are inspected and cleaned, and the volumes in the
chambers of the oil/water separator are gauged. When the volume reaches a preset level, a
vacuum truck pumps out the system and the wastewater is taken offsite for treatment. There has
been no reported discharge from the facility since 2006.

9. Describe the treatment system
List all installed components, including capacities, provide design removal for BOD, T'SS, nitrogen and
phosphorus. If the space provided is not sufficient, attach the description of the treatment system in a
separate sheet of paper.
The treatment works at the facility consists of an ABT, Inc. 512AF Ductile Iron Frame and
Grate installed around the east, west and south sides of the diesel fuel canopy. The trench
drain is installed inside the drip -line of the canopy and sloped so rainfall and/or surface water
does not enter the drain. Flow captured by this grate is discharged through six-inch diameter
PVC piping to a 1,500-gallon capacity, pre -cast concrete oil/water/sand separator. Discharges
from this unit are directed to a 2,000-gallon capacity Kleerwater oil/water separator.
Discharges from this unit are directed through six-inch diameter PVC piping, which ultimately
ties into the existing storm sewer line at the southeast corner of the site. BOD, TSS, nitrogen
and phosphorus removal are not included in the design of this system.
