Unfortunately, the Department only issues one W9 and that is for the Department as a whole. You can send the
payment to the address on the W9. Central Finance will then route to us.

Legally, we can only send payment to the address on the W9. If the address on the W9 is not the correct mailing
address it needs to be changed. If I can send the payment to the mailing address on the W9 then I will do so but
the payment will go and will only go to the address listed on the W9.

Good morning Beth,

A facility has contacted us asking us to provide an updated W-9 form. Apparently the billing address they have on
file for us (DEQ) is diƯerent from the 1617 Mailing Center address that appears on their annual invoices, so they’re
not able to issue a check or make any online payments. I’ve attached both the blank W-9 form they provided me
and a copy of their most recent invoice. Could you help with this, or direct me to someone who can assist?

I still have not received the required documents for us to make the attached payment. I sent the original message
on 5/21, called on 5/28 and still have nothing. The payment is due on 6/7 and mail is taking lo9nger than
usual. Please get this over to me asap.

Can you please send me an updated W9 with the correct mailing address so I can update our system. Once this is
received, we can process the payment.
