I am sorry that I was notable to get back in touch with you yesterday. Following the referenced permit, such
notifications should be followed by a written report within 7 days of the event(see below). Please be sure to
provide all of the information required. You already provided some, but not all, of this information. You can
provide this either by email, or by written letter. Please be sure to indicate when the issue started,and when the
repair was completed.

C-8. Required Notice for Bypass or Upset
After a permittee becomes aware of an occurrence that must be reported,the permittee
shall contact the appropriate Division regional office within the timeframes and in
accordance with the requirements listed in Table 1 below. Occurrences outside normal
business hours may also be reported to the Department's Environmental Emergency
Hotline at(800) 8S8-0368.

Anticipated Bypass Written report at least ten days prior to the anticipated
hvnass. The written report shall include an evaluation of the
anticipated quantity,quality,and effect of the bvnass.
Unanticipated Bypass or Upset Oral or electronic notification within 24 hours of the event,
and
Written report within 7 calendar days of the event. The written
report shall include an evaluation of the quantity,quality,and
effect of the bypa.5

Good afternoon Jerry,
I am contactingyou on behalf of the Gold Bond Building Products Mount Holly Plant(NCG070175)to report an
upset within our stormwater system that has resulted in an unanticipated bypass of a stormwater control
measure.Yesterdaywhile routinely cleaningthe filter screen in our#2 retention pond, personnel from the plant
lost their grip on the filter screen and it fell puncturingthe plastic pipe leading into the outfall#2.This puncture has
allowed water from the pond to free flow through the pipe and into outfall#2.The water that is flowing out of
outfall#2 is coming directly out of the retention pond and water quality is not a concern, but the water is flowing
out of the pond at a faster rate than it was designed for so we wanted to report the upset to the NCDEQ.
Repairs are underway to replace the pipe and we expect the issue to be resolved tomorrow and we will inform you
when the repairs have been completed.
