Division of Water Resources’ Central and Regional staff have reviewed the application package
received on April 1, 2024. However, the Division requires additional information before completing our
review. Please address the items on the attached pages no later than the close of business on July 5, 2024.
Please be aware that the Applicant is responsible for meeting all requirements set forth in North
Carolina rules and regulations. The Applicant is also responsible for any oversights that occur during the
review of the subject application package. The Division may return the application as incomplete pursuant
to 15A NCAC 02T .0107(e)(2) if any omissions are made when responding to the outstanding items in
Sections A through P or the Applicant fails to provide the additional information on or before the above-
requested date.
Please reference the subject application number when providing the requested information. The
Applicant shall sign, seal, and date (where applicable) all revised and/or additional documentation and
submit an electronic response to my attention via the Non-Discharge online portal.

1. The subject permit currently has an open annual fee. The Final Permit cannot be issued until the
Permittee has paid the annual fee pursuant to 15A NCAC 02T .0105(e)(2). Please see the attached
invoice for payment instructions. If the Permittee paid the annual fee before receiving this notice,
please provide documentation verifying payment (e.g., receipt, canceled check, etc.).

1. The subject facility had a permit major modification issued on July 16, 2014. Pursuant to 15A
NCAC 02T .0105(d), submit a Site Map pursuant to the requirements in 15A NCAC 02T .0704(d).
These requirements are:
a. A scaled map of the site with topographic contour intervals not exceeding 10 feet or 25 percent
of total site relief and showing all facility-related structures and fences within the wastewater
treatment, storage, and infiltration areas.
b. Soil mapping units shown on all infiltration sites.
c. The location of all wells (including usage and construction details if available), streams
(ephemeral, intermittent, and perennial), springs, lakes, ponds, and other surface drainage
features within 500 feet of all wastewater treatment, storage, and infiltration sites.
d. Delineation of the compliance and review boundaries per 15A NCAC 02L .0107 and .0108.
e. Setbacks as required by 15A NCAC 02T .0706.
f. Site property boundaries within 500 feet of all wastewater treatment, storage, and infiltration
sites.
g. All habitable residences or places of public assembly within 500 feet of all treatment, storage,
and infiltration sites.
Mr. Bryan K. Lievre, Ms. Helen I. Perez, and Mr. Jordan M. Cocanower of the Wilmington
Regional Office (WiRO) conducted a site visit on April 30, 2024. The WiRO specifically requests
that the Permittee properly depicts all nine (9) monitoring well locations on the Site Map and that
the Permittee verifies the accuracy of the monitoring well coordinates.
