Dear Mr. Baker:
A letter of request for Authorization to Construct was received August 4, 1983,
by the Division and final plans and specifications for the subject project have been
reviewed and found to be satisfactory. Authorization is hereby granted for the con-
struction of a 0.00045 MGD wastewater treatment facility consisting of a 1,000 gallon
capacity septic tank, a 396 S.F. subsurface sand filter with distribution box, a
chlorine contact tank with tablet chlorinator, a cascade type aerator, and associated
4-inch diameter piping and appurtenances to serve the Steve Baker residence in Rocking-
ham County.
This Authorization to Construct is issued in accordance with Part III paragraph
C of NPDES Permit No. NC0052124 issued December 1, 1982, and shall be subject to
revocation unless the wastewater treatment facilities are constructed in accordance
with the conditions and limitations specified in Permit No. NC0052124.
The sludge generated from these treatment facilities must be disposed of in
accordance with G.S. 143-215.1 and in a manner approvable by the North Carolina
Division of Environmental Management.
The Winston Salem Regional Office, telephone number 919/761-2351 shall be
notified at least twenty-four (24) hours in advance of backfilling of the installed
subsurface filter system so that an in-place inspection can be made of said system
prior to backfilling. Such notification to the Regional Supervisor shall be made
during the normal office hours from 8:00 A.M. until 5:00 P.M. on Monday through
Friday, excluding State Holidays.
In event the facilities fail to perform satisfactorily in meeting it's NPDES
permit effluent limits, Steve Baker shall take such immediate corrective action as may
be requiredDivision, including the construction of additi
