longer lives here. I apologize for this account not being paid on time but I didn't receive a bill for it, and have
received no notices about it not being paid. Roger handled a lot of the household bills and I have had to learn
about and deal with in the past year. If you would in the future please list this account in my name,I am the

Mr. Hankins:
All NPDES permittees with a Certificate of Coverage under General Permit NCG550000 must
pay an Annual Compliance Monitoring Fee. This requirement is documented in your current
permit at Part IV [page 11]. The 2009 fee for your residence at 145 Forestdale Drive [CoC
NCG550199] has not been paid.
The fee was billed to you via invoice 2008PR012327 on 12/16/2008 with a due date of 1/15/2009.
The total amount owed is $60.00.
Failure to pay the annual fee is grounds for revocation of your permit, as per 15A
NCAC 2H.0105 (b) (4). This letter serves as final notice that the Division will refer the
fees noted above to the North Carolina Attorney General's Office for collection
through the courts unless payment in the amount of$60.00 is received by June 23,
2009.
Make checks payable to NC DENR; include the CoC number and invoice number on the check.
