Dear Mr. Meyers,
Division of Water Resources’ Central and Regional staff have reviewed the application package
received on May 8, 2024. However, the Division requires additional information before completing our
review. Please address the items on the attached pages no later than the close of business on June 20, 2024.
Please be aware that the Applicant is responsible for meeting all requirements set forth in North
Carolina rules and regulations. The Applicant is also responsible for any oversights that occur during the
review of the subject application package. The Division may return the application as incomplete pursuant
to 15A NCAC 02T .0107(e)(2) if any omissions are made when responding to the outstanding items in
Sections A through N or the Applicant fails to provide the additional information on or before the above-
requested date.
Please reference the subject application number when providing the requested information. The
Applicant shall sign, seal, and date (where applicable) all revised and/or additional documentation and
submit an electronic response to my attention via the Non-Discharge online portal.

A. Overall:
1. Per Form: HRIS 06-16, the Proposed Upgrades section of the Project Narrative is not consistent
with the rest of the application, such as the Site Map and Cover Letter.
a. The Cover Letter does not describe any requested permitting action.
b. The Site Map indicates the following actions:
i. Removal of the existing traveling bridge filter and replacement with a disk filter.
ii. Removal of the decommissioned WWTP.
iii. Replacement of the splitter box and bar screen.
c. The Proposed Upgrades section of the Project Narrative indicates the following actions:
i. Increasing intake capacity is proposed as part of the modifications.
ii. Removal of the existing traveling bridge filter and replacement with a disk filter, with
emphasis on certification of the mechanical and electrical components.
Please verify and revise the application for consistency. Additionally, please provide details on how
intake and/or treatment capacity will be increased (i.e., through the disk filter alone or other
actions).

setback waiver for Parcel No. 0016000001A0000 (Global PIN 8030-19-9824), signed by Paasch
Development, LLC and recorded in the Currituck Register of Deeds.
2. Per Form: HRIS 06-16 Instruction D, 15A NCAC 02T .0706, and 02U .0701, please provide a new
setback waiver for Parcel No. 0015000084I0000 (Global PIN 8030-19-6072) (previously signed
by Raymond Gottlieb) to be signed by Currituck Water and Sewer, LLC and recorded in the
Currituck Register of Deeds.
G. Engineering Plans:

I. Engineering Calculations:
1. Per Form: HRIS 06-16 Instruction K, 15A NCAC 02T .0704(c)(3), 02U .0201(c)(3), there were no
calculations provided on the replacement of the traveling bridge filter with the disk filter. Please
verify and revise with calculations sealed by a Professional Engineer.
J. Site Map:

K. Power Reliability Plan:
1. Per Form: HRIS 06-16 Instruction M and 15A NCAC 02T .0705(k), the provided Power Reliability
Plan does not have documentation of an automatically activated standby power supply onsite.
Please verify that the existing generator can power the essential units after the proposed
modifications and revise, if applicable.
L. Operation & Maintenance Plan:
1. Per Form: HRIS 06-16 Instruction K and 15A NCAC 02T .0707, please note that the issued permit
will require a final O&M Plan via a Compliance Schedule. No response is required.
M. Residuals Management Plan:
1. Per Form: HRIS 06-16 Instruction O, 15A NCAC 02T .0704(j), and 02T .0708, the provided
Residuals Management Plan does not have a detailed explanation of residuals management and
storage requirement based on maximum anticipated production rates. Please verify that the
proposed modifications will not impact residuals management and revise, if necessary.
