The SSO that occurred on 6/18/24 was the result of a bad pump that tripped a
series of breakers that ultimately shut power down inside the control panel,
which prevents the 2nd pump from powering on. The pumps installed are 10 HP
grinder pumps and the pump repair shop informed us the failure looks like it
originated from a failed grinder, which led to a seal fail which led to tripped
breakers. The station is set-up with High-temp and Seal fail inputs but no alarms
were notified in this situation. Our best guess is the failure happened so quickly it
tripped the breakers without acknowledging a seal-fail alarm. The station sent out
an AC power fault instead. So, town crews stopped the SSO because we
responded to an AC power failure but since the failure happened inside the
control panel. It is already past the Automatic Transfer Switch that would have
powered up the standby generator. Standby power would not have alleviated this
problem because the breaker to the pumps were already tripped. Alarms at this
site were active and remain active to alert us in the future of any alarm situations.
Since the failure of this pump seems to have originated in the grinder. As part of a
preventative maintenance plan the town has stocked spare grinders for this pump
so we are able to make in-house repairs to this pump. We plan to inspect and
change (if necessary) the grinders at least every 6-9 months.
