Directions to Site: Take Hwy 109 South out of Winston-Salem and then turn left onto Old Greensboro Road (SR 1756) . Bear right onto West Lexington Avenue Extension (SR 1755) and then turn right onto Chestnut Street Extension (SR 1757) . Proceed to Laura Lane (SR 2927) , turn left, and the O'Connor SFR is the first home on the right. 6 . Discharge Point(s) - List for all discharge points Latitude: 35° 57 ' 05" Longitude: 80° 04 ' 01" Attach a USGS Map Extract and indicated treatment plant site and discharge point on map. USGS Quad No. D-18-NE or USGS Quad Name High Point West 7 . Size (land available for expansion and upgrading) : There is adequate space available for upgrading. 8 . PART II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS 1. Type of wastewater: 100% Domestic a. Volume of Wastewater: 450 gal . (Design Capacity) b. Type of treatment (specify whether proposed or existing) : The exiting system consists of a 1000 gallon septic tank, 391 ft subsurface sandfilter, tablet chlorinator and chlorine contact tank. The system discharges into a ditch approximately 40 yards from Payne Creek. 5 . Sludge handling and disposal scheme: Solids are pumped from the septic tank by local haulers as necessary for disposal in the nearest POTW. 6 . Other: PART IV - EVALUATION AND RECOMMENDATIONS Observations of the system on 6/16/92 revealed that the sandfilter system was functioning adequately. The effluent pipe empties into a side ditch approximately 20 feet from the single chlorinator tube which is located under a tree. The pipe outlet is submerged under the standing water in the ditch. Payne Creek is located approximately 40 yards from the effluent pipe. EFFLUENT LIMITATIONS AND MONITORING REQUIREMENTIS Final NPDES No. NC0051233 During the period beginning on the effective date of the Permit and lasting until expiration, the Permittee is authorized to discharge from outfall(s) serial number(s) 001.
