In accordance with the application for a discharge permit received on July 2, 1996, the Division is
forwarding herewith the subject NPDES permit. This permit is issued pursuant to the requirements of North
Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the US
Environmental Protection Agency dated December 6,1983.
If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable
to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following
receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the
North Carolina General Statutes, and filed with the Office of Administrative Hearings, Post Office Drawer
27447, Raleigh, North Carolina 27611-7447. Unless such demand is made, this decision shall be final and
binding.
Please take notice this permit is not transferable. Part 11, EA. addresses the requirements to be followed
in case of change in ownership or control of this discharge.
This permit does not affect the legal requirements to obtain other permits which may be required by the
Division of Water Quality or permits required by the Division of Land Resources, Coastal Area Management
Act or any other Federal or Local governmental permit that may be required.

in accordance with effluent limitations, monitoring requirements, and other conditions set forth in
Parts I, II, III, and IV hereof.
This permit shall become effective February 1, 1997

SUPPLEMENT TO PERMIT COVER SHEET

Salem Quarter Wastewater Treatment Plant
is hereby authorized to:
1. Continue to operate an existing 0.06 MGD wastewater treatment facility consisting of a bar
screen, equalization basin, dual aeration basins, dual clarifiers, dual sand filters, clearwell,
mudwell, chlorinator and contact basin, dechlorinator, sludge holding tank, and standby
power located at Salem Quarter Wastewater Treatment Plant, Belews Creek Road,
Walkertown, Forsyth County (See Part III of this Permit), and
2. Discharge from said treatment works at the location specified on the attached map into an
unnamed tributary to Belews Creek which is classified as Class C waters in the Roanoke
River Basin.

All stream samples.shall be grab samples and shall be conducted weekly.
** The daily average dissolved oxygen effluent concentration shall not be less than 5.0 mg/l.
The pH shall not be less than 6.0 standard units nor greater than 9.0 standard units and shall be monitored weekly at the effluent by grab sample.
There shall be no discharge of floating solids or visible foam in other than trace amounts.

This facility just began discharging in April of this year. No changes are recommended with
this renewal except to remove some of the special condition language which pertained to
issuance of an Authorization to Construct permit. Effluent temperature monitoring will be
changed from weekly to daily (consistent with Class II WQ limited facility).

PART I - GENERAL INFORMATION

5. Directions to Site: Take Highway 158 east out of Winston-
Salem. Cross through the Hwy 66 intersection and then
proceed 0.7 mile to Belews Creek Road (SR 1965). Turn left
and the Salem Quarter development is located 1.6 mile on the
right. Turn into the subdivision on Salem Quarter Drive and
then turn right on Rangecrest Drive. The WWTP is at the end
of the road beside the creek.

X Yes. The development site consists of 94 acres.

is situated well above the U.T.

present, is located along Salem Quarter Road approximately
1000 ft. NW of the WWTP.
10. Receiving stream or affected surface waters: U.T. to Belews

C. Describe receiving stream features and pertinent
downstream uses: The U.T. flows through a rural area
before its confluence with Belews Creek. The drainage
area upstream of the outf all is 0.97 MZ with a 7Q10 of
0.1 cfs.
Part II - DESCRIPTION OF DISCHARGE AND TREATMENT WORKS

substantially constructed wastewater treatment
facilities; Please provide a description of proposed
wastewater treatment facilities: The 0.06 MGD facility
consists of the following: bar screen, an aerated
15,000 gallon flow equalization basin with two pumps,
flow splitter unit, two 30,000 gallon extended aeration
basins, an aerated 15,000 gallon sludge holding tank,
two 5,000 gallon clarifiers, tertiary filter system
(1.0 gpm/ft2), a 1,250 gallon chlorine contact chamber
with two tablet chlorinators, a 400 gallon
dechlorination chamber with two tablet (sodium sulfite)
dechlorinators, Stevens flow recorder with 300 V-notch
weir and outfall pipe to U.T. to Belews Creek.

1993 was thoroughly addressed in the public hearing held

hearing held 8/21/95.

PART IV - EVALUATION AND RECOMMENDATIONS
The Salem Quarter subdivision is planned to consist of
approximately 140'units served by the 60,000 gpd package plant.
At present, approximately 26 single family residences are
occupied or are under construction at this time. The WWTP first
began discharging in April 1996.
