In accordance with the application for a discharge permit received on December 3, 1998, the Division is
forwarding herewith the subject NPDES permit. This permit is issued pursuant to the requirements of
North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina
and the U.S. Environmental Protection Agency dated December 6, 1983, and as subsequently amended.
Please note the following changes from the previous permit (as indicated in the draft) have been finalized:
Monitoring for Total Residual Chlorine (TRC) 2/week has been added, with a footnote
exempting facility from monitoring as long as chlorine is not used for disinfection.
Monthly monitoring for total nitrogen and phosphorus has been added. This change is consistent
with an effort to gather more data about nutrient loading in this portion of the watershed.
Permitted now has been reduced from 0.140 MGD to 0.100 MGD. This flow limit accurately
reflects the capacity of the existing wastewater treatment facility, built in accordance with the
Authorization to Construct (ATC) issued December 30, 1996. A second effluent page,A(2)., reflects
ultimate flow of 0.140 MGD (for which the facility was originally permitted). Please note that a TRC
limit of 28 µg/1 applies if chlorine is used for disinfection.
Change of Ownership
Please note an additional change. The modification request for change of ownership was received March
15, 1999. The permit has been transferred to Heater Utilities, Inc. from Salem Glen Golf Club, LLC,
which is reflected in the final permit.
If any parts, measurement frequencies, or sampling requirements contained in this permit are
unacceptable to you, you have -the right to an adjudicatory hearing upon written request within thirty (30)
days following receipt of this letter. This request must be in the form of a written petition, conforming to
Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative
Hearings, Post Office Drawer 27447, Raleigh, North Carolina 27611-7447. Unless such demand is made,
this decision shall be final and binding.

Please take note that this permit is not transferable. Part II, E. 4. addresses the requirements to be
followed in case of change in ownership or control of this discharge. This permit does not affect the legal
requirements to obtain other permits which may be required by the Division of Land Resources or any
other Federal or Local governmental permit that may be required.

This permit shall become effective July 1, 1999.
This permit and authorization to discharge shall expire at midnight on December 31, 2003.

1. Continue to operate a 0.100 MGD wastewater treatment facility consisting of an
influent flow control chamber with bar screen, a flow equalization tank, dual 50,000-
gallon aeration basins with course bubble diffusers, dual rectangular clarifiers with air
lift sludge wasting and return, ultra -violet (UV) disinfection, a tablet chlorination and
dechlorination tank for back-up, effluent clear well, effluent pump station, and a flow
measurement device, located at Salem Glen, Dock Davis Road, Clemmons, Davidson
County, and
2. After receiving an Authorization to Construct (ATC) from the Division of `'Water
Quality, expand to a 0.140 MGD ,"rastewater treatment facility, and
3. Discharge from said treatment works at the location specified on the attached map
into the Yadkin River, which is classified as WS-IV waters in the Yadkin -Pee Dee
River Basin.

be submitted at a later date. This action will provide sufficient time to prepare an EAA investigating
options for the additional flow without delaying the renewal of this permit.

Additional Comments
• There are two (2) impaired waters in this sub -basin, but the majority of streams are support -
threatened. This segment of the Yadkin River is supporting its uses, however. Many of the streams
in this sub -basin suffer from impacts of sedimentation.
• This discharge is located approximately six (6) miles upstream of Davidson Water, Inc.'s water
supply intake. In 1994, DEH objected to the proposed permit until an extensive soils investigation
could be conducted and provided DWQ sufficient justification that discharge was the best option.
• Facility also applied for an expansion of permitted flow to 0.20 MGD to accommodate additional
proposed homes; however, applicant did not submit an Engineering Alternatives Analysis (EAA)
demonstrating that upgrade of the existing plant would be the most economically feasible of
environmentally sound options for the additional 0.10 MGD. Thorough information is especially
important given DEH's original reservations about the project. After talking with the Consultant
(John Phillips), the applicant will submit a request for expansion at a later date, since the level of
detail required in the EAA would delay the renewal schedule.

Wastewater discharge from Salem Glen Golf Club (Clemmons, Davidson County) to
the Yadkin River in the Yadkin -Pee Dee River Basin.
15A NCAC 2B .0300 Index #: Yadkin River classified as WS-IV (#12-93.5) in 8/3/92.
ATC Information:
• 12/30/96 ATC issued for a 100,000 gpd package plant.
Basinwide Plan Information:
• This sub -basin . (03-07-02) contains 2 impaired waters listed on the '98 303 (d) list.
The discharge is very close to sub -basin 03-07-04. The closest ambient monitoring
station is USGS 02116500, but it's a fairly good distance down the Yadkin (in sub -
basin 04).
• According to the basin plan, this segment of the river is supporting its uses.
• Many streams in this sub -basin are impacted by sedimentation.
Correspondence:
• Formerly "Sandtree Subdivision" (Mod. on 10/16/96).
• 8/4/94 Memo from Richard Rowe encouraged a more in-depth soils analysis by
pointing out that Davidson Water, Inc. has its sole water supply intake about 6
miles downstream of the (then proposed) discharge.
• 10/11/94 Memo to Public Water is
raised the issue that ifa permit is not
issued because further soils evaluation studies are necessary, there s no mechanism
within DWQ with which to conduct a study; the applicant must turn to a private
registered soil scientist. (i.e., the burden of providing this info. is with the
applicant, not the Division.)
• S. Wilson's 5/18/94 permit notes: DEH's objections to the permit were slow to be
resolved. Finally on 11/22/94, C. Sullins resolved the matter with DEH (L.
Sewall).
DMR Review:
• None. Operation not scheduled until January, 1999.
EAA:
• Add info. requesting EAA sent 1/12/99. Applicant failed to address costs of
expanding the WWTP an additional 0.10 MGD and using non -discharge
alternatives to handle the additional flow (more houses proposed for the
subdivision).
• John Phillips (Consulting Engineer) phoned 2/3/99: would rather proceed with
renewal at current flow than hold permit u . The level of detail required for the
EAA will take longer than the renewal schedule permits, so permittee will submit a
request for expansion at a later date.
General Comments:
Given history, DEH objections to expansion are anticipated. EAA should defend
the need for additional permitted flow; or expansion cannot be granted at renewal.

Dear Mr. Brown:
The Division received your permit renewal application and renewal fee on December 3, 1998. A letter
authorizing Diehl & Phillips to act as your Authorized Representative was also received on December 9,
1998.
A review of the application indicated the facility is requesting to expand permitted flow from 0.14 to 0.20
MGD to accommodate additional proposed homes. The cover letter stated that the original engineering
alternatives analysis (EAA) should support direct discharge as the most feasible alternative if more homes
are served; however, the EAA submitted in 1993 addresses treatment and direct discharge of 0.14 MGD.
In accordance with the Authorization to Construct issued December 6, 1994, the existing plant is
designed for 0.10 MGD. Therefore, an EAA examining the cost of expanding the existing plant from
0.10 to 0.20 MGD and the cost of non -discharge alternatives for the additional flow must be prepared.
This requirement is consistent with T15A: 02H .0100 NPDES application requirements and Division
policy for expansion requests.
Please submit the additional information no later than February 8, 1999. An EAA guidance document is
enclosed to assist you in preparing the report. If you have any questions or comments concerning this

Salem Glen Golf Club, LLC hereby requests the referenced
discharge permit be modified and renewed. To support this request you
will find enclosed:
1. Completed Short Form IUD" application, signed and submitted
in triplicate.
2. Processing fee check for $400.00.
3. A narrative description of the sludge management plan.
4. Site map.
A 100,000 gpd treatment plant has been constructed, but it is not yet
in operation because it only serves a small number of homes. They are
currently operating a pump and haul system (Permit No. WQ0015118).
The permittee has a contract with Heater Utilities to begin operating
the plant in January 1999.
The permittee also requests the referenced NPDES discharge permit
capacity be increased from .14 MGD to .20 MGD. This request is made
to provide sewer service to the number of homes now projected for
construction in Salem Glen. The original alternatives analysis that
supported the permit issuance would continue to support the discharge
alternative if more homes are served.
