This permit is issued pursuant to the requirements of North Carolina General Statute 143-215.1 and the Memorandum of Agreement between North Carolina and the U.S. Environmental Protection Agency dated October 15, 2007 (or as subsequently amended). Please be advised that Permit Condition A. (2.) requires compliance withing thirty (30) days after issuance of this final permit. If any parts, measurement frequencies or sampling requirements contained in this permit are unacceptable to you, you have the right to an adjudicatory hearing upon written request within thirty (30) days following receipt of this letter. This request must be in the form of a written petition, conforming to Chapter 150B of the North Carolina General Statutes, and filed with the Office of Administrative Hearings (6714 Mail Service Center, Raleigh, North Carolina 27699-6714). Unless such demand is made, this decision shall be final and binding. Please note that this permit is not transferable except after notice to the Division. The Division may require modification or revocation and reissuance of the permit. This permit does not affect the legal requirements to obtain other permits which may be required by the Division of Water Quality or permits required by the Division of Land Resources, the Coastal Area Management Act or any other Federal or Local governmental permit that may be required. As of this permit issuance, any previously issued permit bearing this number is no longer effective. Therefore, the exclusive authority to operate and discharge from this facility arises under the permit conditions, requirements, terms, and provisions included herein. Aqua North Carolina, Inc. is hereby authorized to: 1. Continue to operate an existing 0.10 MGD extended -aeration package wastewater treatment system with a discharge of treated domestic wastewater consisting of: a. an effluent flow control chamber with bar screen. b. a flow equalization tank, c. two (2) 50,000 gallon aeration basins with coarse air bubblers, d. two (2) rectangular clarifiers with air sludge wasting and return, e. UV disinfection with backup tablet chlorination/dechlorination, f. Effluent clear well, g. Effluent pump station and h. Flow measuring device. 2. The facility is at the Salem Glen WWTP located on Dock Davis Road near Clemmons in Davidson County. 3. After receiving Authorization to Construct from the Division of Water Quality, expand the facility to a 0.20 MGD WWTP. 4. Discharge from said treatment works at the location specified on the attached map into the Yadkin River, classified as WS-IV waters in the Yadkin -Pee Dee River Basin. Permit NCO083925 A. (1.) Also attached are two copies of this cover letter. There have been no major modifications to the wastewater treatment facility since issuance of the last permit. However, Aqua NC still anticipates expansion of this facility and therefore requests that the renewed permit contain the same language regarding this matter as is in the current NPDES permit. The current permit provides for expansion to 0.2 MGD (upon approval of an authorization to construct) and sets forth effluent limitations for the expanded facility. (Western Region -High Point) No sludge will be treated on any wastewater treatment plant site operated by Aqua North Carolina, Inc. Where practical, sludge removal from Aqua North Carolina West facilities will be transported via a contract hauler to another Aqua North Carolina West facility for the purpose of "seeding" a new or under -loaded plant when there is a need for such. Unusable (or "dead") sludge will be removed by a permitted/licensed contract hauler and properly disposed of in accordance with NCGS 143-215.1. Contract haulers used by Aqua North Carolina West will be required to report, via a sludge -hauler .manifest, the quantity of sludge transported and identify the location of the proposed disposal site if the sludge is not taken to an existing plant operated by Aqua North Carolina West. Aqua North Carolina West has not entered into any agreement to accept sludge into its facilities from plants not owned by them and have no intentions of doing so. Aqua North Carolina West will keep records on the quantity of sludge removed from each facility, the name of the contract hauler, and the destination of the sludge (whether used in another plant or disposed of). The information will be kept on file and will be made available to any regulatory agency having jurisdiction over sludge treatment or disposal. Aqua North Carolina West includes all of the facilities under the jurisdiction of the Winston-Salem Regional Office.
