The assistance of the Operator in Responsible Charge (ORC), Billy Young, was greatly appreciated. All seven facilities are located in Surry County. Keep copy of the plant's certificate of coverage on site at all times; d. Repair the broken dechlor tube; and, e. Outfall location unknown. Section D: Summary of Finding/Comments (Attach additional sheets of narrative and checklists as necessary) On June 20th, 2024, Ron Boone and Michael Gearran of the Winston-Salem Regional Office of the NC Division of Water Resources conducted a compliance evaluation inspection of the Bannertown Hills Subdivision Well #2 and its discharge certificate of coverage, NCG590027, under the General NPDES discharge permit NCG590000. The assistance and cooperation of the operator, Billy Young, was greatly appreciated. The inspection findings are detailed below: a. Flow is calculated using flow restrictors (flow rate * time); b. Facility has four green sand filters and they backwash two at a time, each pair alternating every other day; c. Caustic Soda used for pH adjustment; d. Sodium hypochlorite used for disinfection; e. Sodium sulfite tablets used for dechlorination; f. Water operator is responsible for adding dechlorination tablets because he is at the facility much more frequently than the wastewater operator; g. Four of four dechlorination tubes were in use at the time of the inspection; h. Facility has a monitoring waiver for upstream hardness due to there being no upstream water body; i. Location of outfall is known; j. Operations & Maintenance (O&M) and Visitation Logs were readily available, complete and current; k. Laboratory records and DMR records were reviewed post -inspection and no errors or discrepancies were noted; I. Facility uses Aqua's field laboratory certification #5035; m.
