PURPOSE OF THE FACILITY OPERATIONS, MAINTENANCE, & SPILL RESPONSE PLAN
This plan was developed to comply with 15A NCAC 02U .0801 and describes the necessary
operations and maintenance activities at the referenced wastewater treatment and disposal
facilities.
The field copy of the Facility Operations, Maintenance, & Spill Response Plan has been created
from Aqua North Carolina, Inc.'s Emergency Response Plan prepared in accordance with the
Public Health Security and Bioterrorism Preparedness and Response Act of 2002, specifically
Public Law (PL) 107-188, Title IV. The comprehensive Emergency Response Plan is filed in each
Aqua North Carolina Service Area Office and is reviewed by personnel in accordance with
applicable regulations and company policy. The Emergency Response Plan ensures that AQUA
North Carolina is capable of a standardized, immediate reaction to a variety of both natural and
manmade emergency situations that could arise in their privately -owned water and wastewater
systems.

FACILITY INFORMATION
The Governor's Club Wastewater Treatment Plant (WWTP) is a reclaimed water facility with a treatment
capacity of 300,000 GPD using extended aeration processes and a disposal capacity of 234,128 GPD
through dedicated spray irrigation. Treatment units include:
• Headworks
• Two 30,000 gallon equalization tanks
• Three aeration basins, two 75,000 gallon basins and one 150,000 gallon basin
• Three clarifiers with a total capacity of 20,830 gallons
• Two chlorine contact chambers with tablet chlorination
• Two dual tertiary filters (35 ftZ cells) with air scour
• Two filter backwash wells
• Ultraviolet (UV) disinfection
• Two aerobic digesters, one 45,000 gallon and one 27,000 gallon
• One 1.5 million gallon (MG) 5-day upset pond
• One 15.5 MG irrigation pond (Storage Pond A)
• One 15.0 MG irrigation pond (Storage Pond B)
• 195.68 acres of dedicated spray irrigation operated by Governor's Club, Inc.

OPERATIONAL REQUIREMENTS
Aqua North Carolina, Inc. will employ appropriately certified wastewater treatment plant operators to
operate, monitor, and maintain the treatment process and equipment. General operational
responsibilities include, but are not limited to the following:
1. Comply with all applicable NCDEQ rules and regulations.
2. Operate, monitor, and maintain the treatment facilities.
3. Maintain required records of treatment operations, maintenance, effluent flow, and pond
freeboard, and sludge hauling.
4. Perform required compliance testing in accordance with the permit.
5. Submit monitoring and other reports as required by NCDEQ and in accordance with the permit.
6. Comply with all applicable safety rules and regulations.
7. Comply with the provisions outlined within the Spill Response Plan section of this document.
8. In the event of an Emergency, enact the protocols outlined within Aqua North Carolina's
Emergency Response Plan and notify the Emergency Contacts and appropriate authorities.

OPERATIONS TASK CHECKLIST
Daily:
• Check and clean bar screen.
• Rags and debris cleaned off of the bar screen are to be
bagged and place in the on -site dumpster.
Headworks • Visually check Huber screens for proper operation.
Weekly:
• Powerwash or rinse automatic bar screens, if needed.
Annually:
• Have serviced according to manufacturer recommendations
Daily:
• Check tank levels.
• Make sure flows are in range of normal operations.
Weekly:
• Check that floats and alarms are working properly.
Equalization Tanks Monthly:
• Clean mats out, as needed.
• Check amperage on EQ pumps.
Annually:
• Clean out grit and build up, if needed.
Daily:
• Check DO and pH.
• Perform 30-minute settleometer test.
• Visually monitor for foam build up.
Aeration Basins • Visually inspect mixers and/or diffusers for proper operation.
Monthly:
• Clean stationary DO probes.
Annually:
• Take out diffusers to clean and inspect for wear and tear.
Daily:
• Check returns and skimmers for proper operation.
• Net off any floating debris.
Clarifiers • Check air lift returns for proper operation.
Monthly:
• Check chain drives, motors, and lube, if needed.
• Clean scum baffles, if needed.

Monthly:
Chlorination • Inspect and confirm unit is operational for emergency use.
Daily:
• Make sure you have proper flow to avoid back up.
• Tertiary Filters Check frequency of backwash events in past 24 hrs,
investigate any anomalies, if found.
Monthly:
• Manually backwash to ensure proper operations.
Daily:
• Check indicator light to ensure bulbs are working properly.
Weekly:
• Ultraviolet Disinfection Clean out trough and wipe down sleeves with phosphoric
acid.
Annually:
• Replace UV bulbs every 15 months.
• Replace sleeves, as needed.
Daily:
• Aerate and decant, as needed.
Aerobic Digesters Monthly:
• Haul sludge as needed once reaching 2% solids.
Daily:
• Observe blowers for any inconsistent audible noises.
Monthly:
Process Blowers • Check belts, filters, and oil.
• Grease as required by manufactured recommendations.
• Check amperage on blowers.
Daily:
• Check to make sure pumps are operating properly.
Process Pumps • Confirm that the pumps are not clogged with rags and debris.
Monthly:
• Check amperage on pumps.

MAINTENANCE REQUIREMENTS
Maintenance of the wastewater treatment and disposal system generally includes, but is not limited to
the following:
1. Provide preventative maintenance of equipment as required per manufacturer's instructions
including valves, pumps and motors, chemical feed systems, headworks screen system, blowers,
instrumentation and controls, samplers, and flowmeters.
2. Document all maintenance activities in the on -site Maintenance Log.
3. Maintain necessary tools and supplies on -site for proper repair, maintenance, and calibration of
equipment and controls to operate process equipment and systems.
4. Perform field sampling and testing in accordance with the permit and company policy.
5. Repair any broken equipment or leaks, as needed.
6. Keep spare parts on hand for emergency use and repairs.
7. Ensure that screenings are removed from the facility and disposed of at a sanitary landfill.
8. Coordinate pickup and removal of sludge by a North Carolina licensed sludge hauler and handler
for ultimate residuals disposal.
9. Provide general housekeeping and grounds maintenance.
10. Prepare a preventative maintenance schedule that identifies any upcoming capital needs.
SAMPLING AND MONITORING PLAN
All Division -required monitoring, necessary to evaluate the treatment facility's impact on the
groundwater and surface water, will be conducted and reported by Aqua North Carolina. Required
analysis of effluent, groundwater and surface water parameters will be completed by a division -certified
laboratory. All parameters listed on the current effective permit, applicable to the facility, will be
sampled, analyzed, and reported at the corresponding frequencies.
Records will be retained to comply with 15A NCAC 02T .0108(c). These will include, but not limited to,
reclaimed water irrigated, freeboard measurements, and NDMRs.
Any modification or reissuance of the permit, the sampling requirements will be reviewed by operations
and compliance staff members. The sampling plan will be adapted to reflect any changes upon new
effective date.

RESIDUALS MANAGEMENT PLAN
The Governor's Club Wastewater Treatment Plant generates screenings and residuals as part of the
treatment processes. Screenings removed by the headworks fine screen are to be collected in a
dumpster and removed from the facility for disposal at a sanitary landfill. The residuals generated by the
extended aeration and clarifier processes are aerobically digested and stored in one of the on -site
aerated sludge holding tanks. The holding tanks are regularly emptied by a licensed sludge hauler and
handler for off -site residuals disposal.
Records of sludge hauling activities including the name of the residuals hauler, the residuals hauling
date, and the volume of residuals removed are kept in the on -site facility log book.

SAFETY PROVISIONS
Public Access to the wastewater treatment facility, five-day upset pond, and irrigation ponds is restricted
by either perimeter fencing or locking gates. Personnel is required to wear appropriate Personal
Protection Equipment (PPE) when on facility property. PPE in designated areas may include, but is not
limited to, safety boots, high visibility vest, safety glasses, work gloves, ear protection, hard hats, and
face shields.
Operable eyewash stations, emergency showers, and lock-out/tag-out devices are present at the facility.

SPILL RESPONSE PROVISIONF
To effectively respond to and remediate sewer spills the following procedure will be implemented by
Aqua North Carolina Inc. personal.
1. Immediately upon discovery or notification of a spill, the Operations Supervisor will be
informed.
2. The operator/utility worker shall take the following steps:
• Stop the spill.
• Contain the spill.
• Clean-up the spill
3. Additional resources, including staff and contractors, can be utilized if the spill cannot be
contained by local team.
4. The Operations Field Supervisor will notify the Area Manager of the incident, who will in turn
alert the Operations Director and Environmental Compliance Director.
5. Notification to NCDEQ, if required, will be done within designated timeframe by Director of
Operations, or their designee.
• Spills over 1,000 gallons to the ground; or any volume reaching surface water
o Verbal notification within 24 hours to NCDEQ Raleigh Regional office at 919-791-
4200. After hours, the Emergency Management should be contacted at 1-800-585-
0368.
• 5-Day Report will be completed and reviewed by the Operations Director and
Environmental Compliance Director. Then submitted to NCDEQ within five days of spill.
