Dear Mr. Wes Bell: On April 18th 2024 Pilot Creek WWTP had a C12 reading of 507.6 due to S02 not feeding to the Effluent stream. Contractors onsite requested #3 system be pumped down to inspect a vault that had a pipe we were wanting to use to feed Effluent water to the Belt Press after #1 system was taken out of service. The S02 suction line was feeding from #3 chlorine contact chamber and had not been switched to pull from #4 chlorine contact chamber for pump suction line. The operator checking during rounds did not notice the Sulfur was not feeding, Effluent samples were collected and during C12 test the Lab tech notified the staff that the C12 reading was too high and we had a problem. S02 system was checked and the pump was found to be hot and not feeding S02, so the operators found the valves were not switched to feed suction to the pump from #4 CCC. Valves were switched and the pump was primed and S02 was being fed to the Effluent minutes after the Effluent tests had been performed by the Lab. The corrective action is that every operator has been shown where the S02 valves are located, and now the valves are checked prior to any switch over of the S02 system. The employee that did rounds that morning was written up, and has since resigned. Lee, 3 aV,ey Lee Bailey ORC/Supervisor City of Kings Mountain 200 Potts Creek Rd.
